Job Summary : The Infectious Disease Nurse Practitioner (NP) is responsible for delivering comprehensive, evidence-based care to patients with acute and chronic infectious diseases across inpatient and outpatient settings. The NP collaborates closely with infectious disease physicians, hospitalists, primary care providers, pharmacists, nursing staff, and other healthcare professionals to evaluate, diagnose, treat, and monitor patients with complex conditions. The role includes hospital rounds, outpatient clinic visits, care coordination, patient education, and quality improvement initiatives. The APRN works closely with sponsoring Physicians, Medical Staff, Management Council, Department Heads, Clinic Manager, Hospital Associates, defined customers, and the general public. Promote high quality, evidence-based care in accordance with current infectious disease guidelines and best practice. Monitor outpatient follow up, laboratory surveillance, and treatment adherence to improve outcomes and reduce avoidable hospitalizations. Patient Satisfaction Prioritizes and revises patient care plan/education according to patient/significant others’ input, initial assessment/diagnosis/clinical response, diagnostic findings, and provides direct patient education, and provides assistance to patients when a referral to a specialty physician, health care provider or community resource is needed. Assesses cognitive and personal assets or limitations of patient/family. Provides appropriate patient education, counseling, and support. Represents the Hospital positively in provision of service to the community independently and at Hospital sponsored or community events. Patient Safety Accountable for assignment, organization, and delegated aspects of patient safety, adjusting patient safety efforts according to developmental stage. Assures that all aspects of care provided by self is accurate, current, and appropriate to growth and developmental stages (infancy through geriatrics as assigned), and agency policies and standards. Assesses and manages comprehensive care and, in conjunction with physician, nursing and other professional services, is responsible to evaluate and risk stratify both well and ill patients. Conducts comprehensive or episodic health assessment to include patient history, related current complaint including psychosocial, past medical/personal history, and provides care for clients appropriate to growth and development. Professional Growth Participates in the peer review process, education, competency testing and evaluation, skill development and provides input to evaluation, documenting performance of staff and students in the clinic and/or lab. Remains informed of current knowledge and trends through continuing education, takes a self-directed approach to obtain learning activities/skills to increase professional competence and practice related to primary area of clinical work and population age, maintains educational record, and presents such for annual review. Provides and accepts formal/informal evaluation feedback constructively to improve workplace issues and peer performance and offers suggestions/options for unresolved issues/problems. Customer Service Meets Hospital identified customer service standards; addresses ethical issues and promotes commitment to co-workers. Exhibits excellent guest relations to patients, visitors, physicians and coworkers; shows courtesy, compassion and respect; conforms to Service Excellence requirements. JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items or baked goods that they themselves prepared 7. Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. 8. Reporting any complaint of pain or other symptoms or changes in the patient's condition to the RN Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues. 9. Assisting in ambulation and exercise as instructed by the hospice nurse or therapist. 10. Provide normal range of motion and transfers/positioning as directed. 11. Assistance with self-administered medications as allowed by state regulations, such as “reminders.” 12. Offering and assisting with bedpans and urinals. Providing assistance as assigned with light laundry needs, emptying waste baskets and other light homemaker tasks. 13. Performing range of motion and other simple procedures as ordered with appropriate completed/demonstrated skills competency. 14. Providing respite for patient's/family/caregiver when on-site, as appropriate. 15. Keeping patient's living area clean and orderly, as assigned. 16. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. 17. Participates in quality assessment performance improvement teams and activities. 18. Respectful of patient and family/caregiver environment and patient's personal needs. 19. As a mandatory reporter, reports failure to comply with the requirements of the Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 20. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. JOB DESCRIPTION SUMMARY The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. FLSA Status Non-ExemptJob Role Summary The Eskenazi Health Clinical Decision Unit is a specialized unit that provides short-term observation and treatment for patients who require further treatment past their emergency department visit. The unit is designed to improve patient flow, reduce unnecessary admissions, and enhance the quality of care for patients. Sixteen beds are staffed by four nurses and a care tech. The patients are cared for by nurse practitioners, and the team works collaboratively to evaluate patients, order and interpret diagnostic tests, and make treatment decisions. The Staff Nurse II provides clinical care and uses coordination skills for every patient served.
