Job Description JOB SUMMARY Under the supervision of the Medical Assistant Supervisor, guided by policies and procedures of the Ambulatory Services, and in collaboration with the Clinical Nursing Staff, Hospital Assistant III assists in the delivery of care to patients in the Bayfront Medical BuildingWork includes a variety of personal care and clinical procedures, including ensuring positive care outcomes. They will perform assigned patient care and laboratory procedures supporting the specialty care services provided to patients within the practices. They are required to learn and be able to assist in the multiple departments across Bayfront.S/he is required to adhere to the Telephone and House Standards as adopted by the Medical Center. This position makes a difference for patients in an outpatient care unit by providing excellent customer service, facilitating and ensuring the accuracy of the information flow between medical, hospital staff and departments to maximize unit efficiency. This Hospital Assistant III functions in the outpatient clinical settings at all UCSF Health sites (Parnassus, Mt. Zion, joint UCSF health partnerships, Satellite Clinics, or with UCSF Faculty atother off-site clinic sites, et al). REPORTING RELATIONSHIPS Supervises no staff. Qualifications REQUIRED QUALIFICATIONS 6 months MA experience Certification from an approved Medical Assistant program or equivalent combination of training and experience. Knowledge and abilities are essential to successfully performing the duties assigned to the position. Current American Heart Association CPR Certification. Demonstrated proficiency with computer usage, keyboard skills, and software programs such as Microsoft Word and Excel. Ability to work effectively with people from diverse cultures and diverse socioeconomic situations. Preferred Qualification 6 months experience with OHNS, Urology or similar in an ambulatory setting/medical practice. CPT-1 Phlebotomy License preferred Proficiency with EPIC (APeX) electronic health records system. Bilingual or multi-lingual capability (Spanish, Cantonese, Russian) strongly preferred. Experience collecting and entering data, identifying errors and missing information, and performing fundamental process improvement analysis. Experience with insurance authorizations and interacting with retail pharmacies to refill medications. Job Description The University of California, San Francisco (UCSF) Lung Transplant is seeking a full-time Advanced Practice Provider (APP) to join their Advanced Lung Disease Service (ALDS). UCSF is ranked #8 nationally in pulmonology and lung transplant has had significantly higher-than-expected survival rates for post-lung transplant recipients for more than 13 years consecutively! This accomplishment is a result of a great multidisciplinary team that strives to redefine possible at every level of care. The UCSF team consists of highly experienced physicians, surgeons, APPs, nurses, social workers, pharmacists and many others that provide quality care for patients with acute respiratory distress syndrome (ARDS), Alpha-1 antitrypsin deficiency, bronchiectasis, chronic obstructive pulmonary disease (COPD), cystic fibrosis, idiopathic pulmonary fibrosis, interstitial lung disease and pulmonary hypertension. The APP will be responsible for coordinating the care of patients in the hospital setting Responsibilities Work with physicians/surgeons/APP colleagues and other clinical and administrative staff to ensure efficient patient management based on accepted practice, including but not limited to, taking histories, performing physical examinations, peri-operative planning/coordinating with outside providers and primary care providers (PCPs) as applicable, responding to changes in patient status, providing prescriptions/evaluations/referrals, diagnostic tests/treatments as permitted under core privileges and standardized procedures. Evaluate for and order home health services, durable medical equipment (DME), Department of Motor Vehicles (DMV) certifications, refer for consultations, physical therapy, order radiographic imaging or diagnostic tests, and specialized treatments (e.g. chemotherapy with the approved privilege). Provide patient and family education, arrange follow-up appointments, and engage in any approved privileges after the APP has demonstrated competency through proctoring and FPPE process. To evaluate, assess and create a documented plan of treatment per patient’s presenting HPI or established plan of care as defined by the UCSF Medical Staff Bylaws. To answer clinically relevant patient medical advice calls or during bedside communications or in coordination with health care professionals, patients, and their families as necessary. To independently, or in a shared visit model, evaluate routine, acute, critical, new patients, or specialty consultations when needed or required and discuss with attending and staff as appropriate and document in the EHR. Any required documentation workflow must be adhered to Be able to interpret and act upon ordered tests, procedures, and radiographic studies. To assist in formulating and implementing protocols to improve efficiency in the clinical specialty Effectively communicate with family or patients in person, via telephone and/or via other electronic modalities in a timely manner. The Pediatric Intensive Care Unit (PICU) at UCSF Benioff Children's Hospital provides extraordinary high-level care for children who require continuous monitoring. The unit has 20 beds for infants and children suffering from a variety of conditions or recovering from surgery. More than 1,000 children per year are admitted into the intensive care units at UCSF Benioff Children's Hospital arriving via ambulance, helicopter and airplane from throughout the western United States. The Pediatric Transitional Care Unit (TCUP) is a 18-bed unit for infants and children suffering from a variety of conditions or recovering from surgery. The TCU provides care to patients with a moderate to high complexity of disease such as those recovering from solid organ transplantation, titration of anticoagulant therapy and other infusion, and post-operative specialty surgery care (ENT, neurosurgery). Patients requiring respiratory support including BIPAP, CPAP, high flow nasal cannula and patients requiring mechanical ventilation with tracheostomy in place are also routinely admitted to the TCUP. The Nurse Practitioner will act as a full member of the team taking responsibility for the safe delivery of care in collaboration with attending physicians, fellows and residents. This team member will meet productivity expectations as set forth by the Division Chief, performing any duties related to patient care activities that are appropriate and clearly delineated. Job Description The Pediatric Nurse Case Manager coordinates resources and services in response to individual patient’s needs and fiscal realities, performs utilization review activities and oversees coordination of complex discharge planning for assigned case load. The Nurse Case Manager is an active member of the interdisciplinary team contributing to team goal setting. The Nurse Case manager acts as a consultant to the clinical team, service lines and other departments and participates in program development and quality improvement initiatives. The Nurse Case Manager provides age-appropriate assessments, interpretation of data, and delivery of interventions.
