Nurse practitioners will be responsible for the symptom management of chronically/terminally ill patients within an interdisciplinary team environment. This role will provide disease process and trajectory education to the team, patients and families. The NP will assess, order and interpret clinical testing, diagnose and treat symptoms related to a variety of chronic disease states to focus on palliative symptoms and advanced illness education. Responsibilities Provide expert care to residents on ventilators and with tracheostomies. Monitor ventilator settings, oxygen levels, and respiratory status; respond promptly to alarms and changes in condition. Administer medications and treatments safely and accurately. Collaborate with respiratory therapists, physicians, and interdisciplinary team members to optimize patient outcomes. Document all assessments, interventions, and care in accordance with regulatory standards. Participate in care planning, infection control, and quality assurance activities. Educate residents and families on respiratory care and overall health management. Qualifications Current Registered Nurse (RN) license in the State of New York (required). Minimum of one (1) year of experience in ventilator and tracheostomy care (required). Prior experience in long-term care or sub-acute rehab strongly preferred.
