OVERVIEW: Assist the Director of Nursing and Assistant Director of Nursing in planning, developing, organizing, implementing, evaluating, and directing the day-to-day functions of the Nursing Department. Maintains quality clinical care for residents/patients in accordance with current rules, regulations, and guidelines that govern the Facility. RESPONSIBILITIES: • Supervises the clinical care planning and delivery processes to ensure the coordination of quality patient care. Reviews and updates the care plan accordingly • Assesses residents/clients upon admission and periodically as conditions change • Performs rounds to evaluate patient condition • Provides or assists with the delivery of direct patient care when indicated, including activities of daily living, meal assistance, medication and treatment administration, and leisure activities when indicated • Obtains and implements Physician Orders, checking for accuracy and completeness • Communicates between shifts regarding changes in resident/patient conditions • Ensures a safe and secure environment for patients/residents, staff and visitors • Provides clinical assessment to MD/NPs on call for quality outcomes delivered to patients • Plans, implements and evaluates the clinical competencies and on-going educational/orientation needs of the staff. • Participate in Facility surveys (inspections) made by authorized government agencies as requested by the Administrator or Director of Nursing Services • Participate in developing, implementing, and periodically updating the resident’s written discharge plan • Ensure that a sufficient number of nursing staff is available for each shift to ensure that quality care is maintained • Make daily rounds of the Nursing Services Department to ensure that all nursing service personnel are performing their work assignments in accordance with acceptable nursing standards. Report findings to the Director of Nursing Services • Review complaints and grievances made or filed by department personnel. Make appropriate reports to the Director of Nursing Services, as required or as may be necessary • Maintain nursing supplies inventory and other responsibilities as required • Assist in the Quality Assurance and Performance Improvement (QAPI) process QUALIFICATIONS: EDUCATION / EXPERIENCE: • Active RN/LPN Nursing License required • Current registration and license to practice as a Registered Nurse in the state employed in • Peripheral IV certification • Minimum 5+ years nursing experience in a hospital, nursing home or other healthcare setting and a minimum 1-year experience as a nurse supervisor • CPR/BLS certified • Strong knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care centers. • The ability to read, write, speak and understand the English language required, knowledge of other language helpful SUPERVISORY RESPONSIBILITIES: • Supervises the nursing personnel and the day-to-day nursing activities of the facility during an assigned tour of duty WORK ENVIRONMENT: • This job operates in a health care setting. Position Overview The Registered Nurse (RN) Supervisor plays a key leadership role in managing nursing operations during assigned shifts at Constant Care Agency’s partner nursing facilities in the Bronx. The RN Supervisor ensures that residents receive safe, high-quality care consistent with the standards of practice, regulatory requirements, and the mission of Constant Care Agency. This position requires strong clinical judgment, supervisory ability, and compassion for the elderly and long-term care population. Primary Responsibilities Clinical Supervision Oversee all nursing and direct care staff (RNs, LPNs, CNAs) on assigned units or shifts. Communicate critical incidents to the DON or Administrator on call. Resident & Family Relations Foster compassionate, resident-centered care and ensure residents’ dignity and rights are upheld. Communicate effectively with residents and families regarding care issues or changes in condition. Participate in care plan meetings when required. Qualifications Licensure: Current, unencumbered Registered Nurse (RN) license in New York State . Experience: Minimum 2–3 years of RN experience , with at least 1 year in a long-term care or skilled nursing facility . Prior supervisory experience strongly preferred. Education: Graduate of an accredited nursing program (BSN preferred). Certifications: BLS required; ACLS preferred. Knowledge: Comprehensive understanding of NYS DOH and CMS nursing home regulations. Familiarity with MDS documentation, infection control, fall prevention, and pressure injury protocols. Skills: Strong clinical assessment and problem-solving ability. Description The Medical Assistant I provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. This individual is also responsible for assisting with clinical, administrative and organizational tasks associated with patient care coordination. Responsibilities 1. Assists the provider during patient visit via obtaining vital signs (height, weight, blood pressure, pain scale etc) and documenting vital sign results and transcribing medication information into the electronic medical record (EMR). 2. Obtains prior authorizations for prescriptions and durable medical equipment 3. Drafts prescription requests in EMR and sends to provider for review, completion and processing 4. Performs Phlebotomy services inclusive of collecting and processing blood, urine and stool specimens and use of a centrifuge for spinning bloods 5. Performs POCT (w/the exception of glucose) and documents results in EMR for provider to review 6. Performs daily Quality Control for all POCT equipment 7. Orders clinical supplies when necessary 8. Maintains and stocks exam and consult rooms with clinical supplies and clerical forms used by Providers 9. Assists providers with procedures by preparing supplies and equipment and acting as a chaperone as needed. 10. Perform EKGs when ordered by provider 11. Performs pre-visit planning for Diabetic and CHF patients as per practice guidelines 12. Monitor and maintains refrigerator temperature logs and biomed expirations for medical equipment 13. Participates in daily huddles with MDs and other clinical team members to review daily care management patient lists and discuss patient's treatment plan & management 14. Performs age-specific screenings for Activities of Daily Living, Falls, Alcohol, Tobacco, and/or Depression screenings annually for every patient 15. Enters data into the electronic medical record related to medication adherence, diabetes control, foot/eye exam compliance for provider review and reconciliation 16. Assists providers with distributing educational materials to patients 17. Assists Population Health Management team by verifying patient has completed their colorectal screening, breast screening, cervical cancer screening and other pertinent preventive screens. 18. Retrieves information regarding hospital admission and Emergency Room visits from outside facilities for evaluation by provider for transition of care coordination 19. Assists providers with follow-up related to patients on Coumadin therapy 20. Assists at the front desk as needed: answer telephones, register patients, discharge patients, schedule appointments, copy charts, schedule ancillary tests, and follow up on overdue diagnostic test results. 21. Communicate critical incidents to the DON or Administrator on call. Resident & Family Relations Foster compassionate, resident-centered care and ensure residents’ dignity and rights are upheld. Communicate effectively with residents and families regarding care issues or changes in condition. Participate in care plan meetings when required. Qualifications Licensure: Current, unencumbered Registered Nurse (RN) license in New York State . IMT participants interact with homeless services, criminal justice, and behavioral health service systems. IMT teams are hired, trained and funded to maximize flexibility and continuity of care for persons who historically have been poorly served. IMT teams are multi-disciplinary and include psychiatry, nursing, social work, peer specialists and other staff members offering rehabilitation, treatment and recovery support services. The team provides expertise in the areas of housing, substance use and harm reduction, criminal justice, peer support and behavioral health recovery.
