JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items or baked goods that they themselves prepared 7. Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. 8. Reporting any complaint of pain or other symptoms or changes in the patient's condition to the RN Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues. 9. Assisting in ambulation and exercise as instructed by the hospice nurse or therapist. 10. Provide normal range of motion and transfers/positioning as directed. 11. Assistance with self-administered medications as allowed by state regulations, such as “reminders.” 12. Offering and assisting with bedpans and urinals. Providing assistance as assigned with light laundry needs, emptying waste baskets and other light homemaker tasks. 13. Performing range of motion and other simple procedures as ordered with appropriate completed/demonstrated skills competency. 14. Providing respite for patient's/family/caregiver when on-site, as appropriate. 15. Keeping patient's living area clean and orderly, as assigned. 16. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. 17. Participates in quality assessment performance improvement teams and activities. 18. Respectful of patient and family/caregiver environment and patient's personal needs. 19. As a mandatory reporter, reports failure to comply with the requirements of the Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 20. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. Staff Nurse-MS7-Per Diem

Under general supervision, assumes 24-hour accountability to plan and provide direct professional nursing care for specified patients throughout the entire episode of service delivery as defined by population and unit. Provides nursing services to patients and families in accordance with the standards of practice of a professional nurse, CCMC policies & procedures, TJC, and State & federal regulations. Reports to Nurse Manager. Clinical Practice -On a daily basis, demonstrates clinical expertise and uses the nursing process to provide safe, appropriate patient care to a defined patient population according to CCMC and unit policies and standards. Population Specific - Demonstrates knowledge of the differences and needs of patients in appropriate, specific populations from neonate through adolescence and applies them to practice. Demonstrates cultural sensitivity in all interactions with patients/families. Unit Leadership- Delegates duties and tasks associated with patient care to clinical staff according to hospital policy and their scope of practice. As assigned, contributes to the leadership of the unit by functioning as the Resource Nurse and/or by participating in the professional development of others through preceptor/mentor activities. Supports the unit/department’s work productivity and resource management activities. Documentation/Communication- Completes all required documentation in a timely manner, according to CCMC policies and standards; necessary information is effectively communicated to all members of the health care team. Patient/Family Partnership & Education- Partners with child and family in evaluating outcomes. Monitors, trends, and evaluates patient/family responses to treatment. Provides appropriate education to patients and families regarding all aspects of care according to CCMC policies and standards; supports the organization’s efforts to patient/family education. CCMC Standards- Demonstrates support for the mission, values and goals of the organization through behaviors that are consistent wit h the CCMC Standards. Education and/or Experience Required: ASN required upon hire. BSN preferred upon hire and required within 3 years of hire. Education and/or Experience Preferred: Minimum of 2 years pediatric and/or medical/surgical experience preferred. License and/or Certifications Required: Current State of Connecticut Registered Nurse Licensure required or nurse licensure compact (NLC) and maintained thereafter. American Heart Association, or American Red Cross, Healthcare Provider CPR/BLS certification required upon hire and maintained thereafter Nonviolent Crisis Intervention training certification required within 3 months of hire and maintained thereafter. PALS required within 6 months of hire. Knowledge, Skills and Abilities: Knowledge of: Theory, principles, techniques and practice of professional nursing. State regulations regarding nursing practice. Clinical standards of care; documentation and patient record policies and standards. Teaching techniques; identification of learning needs and provision of patient/family education. Developmental and population-specific patient & family assessment techniques and how to apply to practice. Accreditation and certification requirements and standards. Skills: Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community. Basic proficiency in utilization of personal computers in networked Windows-based health care environment to access related software applications; locate, open, edit and print files and information utilized by the unit; and use of Internet as research tool. Ability to: Work individually or with within teams and/or other organizations within the medical and research community. Make clinical and procedural decisions using the nursing process.
