Atlantic Coast Rehabilitation and Healthcare Center, a 160-bed skilled nursing facility in Ocean County, New Jersey, is seeking an experienced MDS Director (RN) to lead our MDS program. If you have a strong background in MDS, care planning, and regulatory compliance, and enjoy collaborating with an interdisciplinary team, we'd love to hear from you. Key Responsibilities: Manage the scheduling, completion, and accuracy of all MDS assessments using Point Click Care (PCC) . Ensure compliance with all State and Federal MDS regulations . Submit MDS assessments electronically in a timely and accurate manner. Lead and participate in weekly utilization review meetings with the interdisciplinary team. Develop, review, and update individualized resident care plans. Educate and support staff on MDS processes and documentation requirements. Monitor Quality Measures (QM) and implement corrective actions to improve outcomes. Maintain accurate ICD-10 coding for residents. Participate in facility meetings and assist with other clinical responsibilities as needed. Compensation: $90000-$105000 per year and is based on your experience Qualifications : Current New Jersey Registered Nurse (RN) license in good standing (Required) . 1–2 years of recent MDS experience in a skilled nursing facility preferred. Experience using PointClickCare (PCC) requried Strong understanding of Medicare, Medicaid, MDS assessments, RAI process, and care planning. Excellent communication, organizational, and leadership skills. Our 16‑bed ICU/CCU and 10‑bed Stepdown Unit care for a diverse, high‑acuity patient population, including surgical, respiratory, cardiac, oncologic, neurologic, and multisystem conditions. The CCU is supported by an intensivist‑led physician team that provides progressive, research‑driven, and highly engaged clinical partnership. Monitors appropriate usage of speciality and Zonaire beds. Consistently adheres to hospital policy regarding the insertion, maintenance and removal of foley catheters, if applicable. Able to verbalize and demonstrate proper rationale and techniques surrounding all aspects of indwelling urinary catheter care as per policy # 4668. Fulfills the requirements for competencies and completion of annual mandatories surrounding indwelling urinary catheters. Takes a patient assignment when necessary. Qualifications Bachelor's degree in Nursing required. Master's degree in Nursing required or must be obtained within 3 years from date of hire (or internal transfer) as a condition of employment. A minimum of two years experience in ICU/CCU as an RN required. Previous supervisory experience preferred. Current NJ RN License required. BLS & ACLS Certification required. Certification in specialty area preferred. Completion of the specialty specific pharmacology test is required. Strong professional, organizational, and interpersonal skills to effectively relate with all members of the healthcare team.
