Department Overview The CHO Adult Ambulatory Oncology Infusion Clinic at OHSU primarily serves patients within Hematology Oncology, Medical Oncology and Classical Hematology. We collaborate with KCI Ambulatory Oncology Clinics, Dietitians, Pharmacy, Social Work, Case Management and other members of the multidisciplinary health care team to provide exceptional oncology care for our patients and community. Our teams support programs that include, but are not limited to, the Hepatic Arterial Infusion Pump program, Phase I Clinical Trials, and the OHSU Cellular therapy programs. Job Description Summary: Primary responsibility providing nursing services to include on-site referral coordination, patient assessment, education, training, problem solving of new, recycling, hospitalized and current patients. Job Description: RESPONSIBILITIES: Performs an initial patient assessment to determine if the patient is an acceptable candidate for home IV therapy and if the home IV therapy can be safely performed in the home setting. Determines if the patient meets Intramed Plus’ acceptance criteria. Documents the assessment of the patient completely in a timely manner. Gets needed clinical information to assess reimbursement and clinical needs Developes the initial treatment plan with the pharmacist and IP team for initiation of the home infusion therapy. Communicates assessment back to referring agent and IP team. Educates the patient or caregiver about the home IV therapy according to company policy and protocols. Trains the patient or caregiver on the administration techniques and drug delivery device/operation for administering the home IV therapy according to company policy and protocols. Observes and determines when the patient or caregiver has successfully completed the training and then certifies that the patient or caregiver can safely administer the IV therapy in their home setting when applicable. Communicates and documents the education and training process of the patient or caregiver immediately. Educates and trains, if needed, the home health nursing agency’s personnel on the drug administration method if they are accepting the responsibility of administering the IV medication to the patient in the home setting. Begins coordination after first introductory visit. Assists the Nurse Supervisor or designee in the coordination of the home IV therapy for the patient discharge planners, case managers, home health nursing agencies, etc., in a timely manner. Coordinates the initiation of the patient’s home IV therapy with Pharmacy, Operations and Reimbursement areas. Documents the coordination arrangement and activities in a timely manner. Communicates verbally and documents all coordination activities as set up. Routinely monitors the patient’s progress during hospitalization and after compliance during entire home IV therapy. Triages any problems which the patient/caregiver or home health nursing agency’s personnel and provides recommendations to solve the problems. Documents all monitoring activities and communicates effectively with the branch for revisions to the patient’s Care Plan when applicable. Develops and maintains a processional relationship with case managers, social works medical departments and physicians at assigned accounts who have potential needs for home infusion therapy services. Works with hospital discharge managers to simplify and expediate the patients discharge in a safe and timely manner. Does the initial training and admission at the hospital unless circumstances do not allow. Presents a positive and professional image when performing all duties. Assists in identifying potential marketing sources to expand business opportunities. Participates in the assessment of patient care services and identification of service problems which do not meet Intramed Plus’ service goals. Documents any service problems through the appropriate methods established, such as Incident Reports, etc. Assess patient and chart observations of the patient’s condition related to prescribed intravenous therapy, at each visit. Provides skilled nursing care and prescribed intravenous therapies with a variety of patient populations within various potentially complex home situations. Maintains technical skills according to the standards of the State Board of Nursing and Intramed Plus, as measured by competency assessments during orientation and annually. Complete patient evaluation tasks, including reviewing medications and vital signs. Administer prescribed infusions as ordered. Assess Central Venous Access Device and provide appropriate care to include dressing changes and site care. Obtain labs as ordered by the physician. Coordinate with physicians, pharmacists, and other individuals of the multidisciplinary team in the patient’s care. Document and maintains a complete and updated treatment plan at all times. Meets required guidelines for documentation of certification and re-certification periods and obtains verbal orders per protocol as deemed necessary. Description The Hospice Access Field and Triage RN will complete medical requirements while coordinating a whole-person plan of care. Care is according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act. In Field role, the RN will visit patients in their homes (private or facility) to present the hospice program and conduct eligibility assessments, coordinating care with Hospice MD, family and facility caregivers, and interdisciplinary team (IDT). Department Overview This position will support in-clinic both primary care clinics located at Beaverton Clinic and Orenco Station. In addition, this position will support additional primary care clinics within the Family Medicine and Internal Medicine Departments (remotely) if Beaverton and Orenco clinics do not have staffing needs. Function/Duties of Position Primary Care at Orenco and Beaverton clinic serves both Family and Internal Medicine providing comprehensive primary care to patients across the lifespan, including pregnant individuals, infants, children, adolescents, adults, and older adults. Patients are often moderately stable to moderately unstable, with complex medical conditions and significant social determinants of health (SDOH) needs. A portion of nursing work occurs outside of scheduled provider visits. Nursing services include RN visits, telephone triage, obstetric intakes and support, medication administration, patient education, and transitions of care and RN care management. RN visits include assessment and management of acute and chronic conditions, medication and minor procedural care (ex: suture removals), chronic disease monitoring, and preventive follow-up. Nurses also support maternal-child health, substance use treatment, and care management through patient education and coordination. JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items or baked goods that they themselves prepared 7. Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. 8. Reporting any complaint of pain or other symptoms or changes in the patient's condition to the RN Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues. 9. Assisting in ambulation and exercise as instructed by the hospice nurse or therapist. 10. Provide normal range of motion and transfers/positioning as directed. 11. Assistance with self-administered medications as allowed by state regulations, such as “reminders.” 12. Offering and assisting with bedpans and urinals. Providing assistance as assigned with light laundry needs, emptying waste baskets and other light homemaker tasks. 13. Performing range of motion and other simple procedures as ordered with appropriate completed/demonstrated skills competency. 14. Providing respite for patient's/family/caregiver when on-site, as appropriate. 15. Keeping patient's living area clean and orderly, as assigned. 16. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. 17. Participates in quality assessment performance improvement teams and activities. 18. Respectful of patient and family/caregiver environment and patient's personal needs. 19. As a mandatory reporter, reports failure to comply with the requirements of the Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 20. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. JOB LIMITATIONS The hospice aide will not function in any manner viewed as the practice of nursing according to the Nurse Practice Act. POSITION QUALIFICATIONS 1. Completion of a state approved certified nursing assistant course or equivalent training 2. Nursing Assistant Certified (NAC) 3. Current Healthcare Provider CPR (Cardiopulmonary Resuscitation) certification ((may not be internet based unless the demonstration of skills was hands on and observed by a certified trainer) 4. Current Food Handlers Permit 5. High school diploma or General Equivalency Degree (GED) 6. Ability to read and follow written instructions and document care given. 7. Understands hospice philosophy, comfortable providing specialized care to the terminally ill. 8. Prior experience in hospice, orderly, acute , rehabilitative, or long term care facility. 1 year previous home health/hospice experience preferred 9. Basic computer skills to document in electronic medical records (EMR) 10. At least 18 years of age. 11. Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order.

The Knight Cancer Institute Outpatient Clinics deliver comprehensive care to adult ambulatory patients diagnosed with a wide range of cancers, including solid tumors and benign hematology. We are seeking an Advanced Practice Provider to join our Thoracic, Head & Neck Program. Function/Duties of Position Position Summary OHSU classification: Clinical Associate APP , represented by the Oregon Nurses Association & Advance Practice United, subject to the active collective bargaining agreement. This Clinical Associate-APP provides care in conjunction with a team comprised of physicians, APPs, RN coordinators, and other allied health professionals. APPs may practice both independently or in a shared-care model with physicians, as determined by the needs of the team. Additionally, this APP will perform procedures and may intermittently provide oversight to the infusion unit. The ideal candidate will thrive in a fast-paced, team-oriented environment and demonstrate clinical knowledge and oncologic experience to support a complex patient population. The Clinical Associate-APP delivers care aligned with a defined scope of practice in Outpatient Adult Medical Oncology , with work expectations operationalized through departmental workflows, productivity standards, and performance expectations consistent with the collective bargaining agreement and institutional policies. This role is structured to meet contract-defined work expectations, inclusive of daytime coverage, aligned with the clinical coverage model and patient care needs of the service. A fixed schedule will be determined at the time of hire and may include evening, weekend, and holiday coverage.
