The Home Infusion RN Clinician 2 is an experienced capable clinician focused on expanding knowledge and skills. Continuum Home Health and Home Infusion is the home care department of UVA Health. We are an academic home care agency, providing care in the community for over 30 years. The Home Infusion Nurse delivers specialized medications and educates patients in the home settings. This RN, consistently provides effective direct care to patients in their homes for an assigned caseload of patients; works as part of the interdisciplinary team; oversees the care planning and care coordination for a variety of complex patients.
