Overview: Piedmont Healthcare is currently recruiting for a Physician Assistant/Nurse Practitioner to join the Piedmont Pulmonary and Sleep office in Buckhead. This is a full-time outpatient position focusing on pulmonary and sleep medicine patients. Responsibilities: Key Responsibilities: Ambulatory care responsibilities include, but are not limited to: Functions as an independent provider in a busy Pulmonary and Sleep office with MD's available to assist in evaluating complex patients as needed. Takes patient histories, conducts physical examinations, writes progress notes, and develops appropriate treatment plans. Orders and interprets laboratory studies and diagnostic procedures including Chest X-rays, Chest CT's Pulmonary Function testing, and Sleep studies. Overview Hospice Clinical Liaison - RN Location: Atlanta, GA and some of Gwinnett County Find Your Passion and Purpose as a Full-Time Clinical Liaison RN Schedule: M-F / limited on-call What You Need to Know The Clinical Liaison, either a Registered Nurse (RN), or Social Worker, will act as liaison between the contracted hospitals and the hospice team, make inpatient visits to hospice patients in contracted facilities, act as consultant for pain and symptom management, provide in-service education to providers, perform pre-admission evaluations for appropriateness for hospice services, initiates the admission process by educating and informing patient/family of hospice and its services.
