The Medical Assistant is an unlicensed medical professional cross-trained to perform routine administrative and clinical tasks to keep the medical centers running smoothly. The duties of Care Promoter 1 vary from setting to setting, depending on the size, location and type. In addition to administrative duties, Care Promoters perform clinical duties under direct physician supervision in accordance with state medical practice acts. The Care Promoter 1 is an integral member of a Physician-lead Care Team that is focused on providing excellent and comprehensive primary care for a specific population of patients. ESSENTIAL JOB DUTIES/RESPONSIBILITIES: Measures and records patient vital signs; records patient interview and medical history. Catalogs and communicates patient needs to the PCP, Clinician, Specialist and/or other clinical personnel. Prepares patients for examinations and performs routine screening tests; assists physician with exams; explains treatment procedures and physicians' instructions to patient. Observes patients and reports any changes in patient conditions to the PCP, Clinician, Specialist and/or other clinical personnel. Perform non-critical medical and therapeutic procedures based on medical instructions. Collects phlebotomy and other lab specimens; performs basic waived lab tests. Prepares and administers medications; changes dressings, applies bandages, removes sutures and other first aid procedures; uses CPR skills when necessary, all under physician supervision. Maintains supplies, equipment, stocks and sterilizes instruments; practices OSHA safety standards; performs accurate, legal, and ethical documentation at all times. Processes patient phone messages, returning calls and routing them to other team members as appropriate; calls patients to obtain and relay pertinent information for the physician. Upon physician approval and authorization, calls in prescriptions to the pharmacy. Uses communication skills with appropriate medical terminology; and follows appropriate legal and ethical professional conduct; authorized individuals will use the ChenMed Rx system to support physician medication dispensing. Provides health coaching to a defined group of patients to support healthy lifestyle choices. Follows up with coached patients via weekly calls. Performs other duties as assigned and modified at manager’s discretion. JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items or baked goods that they themselves prepared 7. Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. 8. Reporting any complaint of pain or other symptoms or changes in the patient's condition to the RN Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues. 9. Assisting in ambulation and exercise as instructed by the hospice nurse or therapist. 10. Provide normal range of motion and transfers/positioning as directed. 11. Assistance with self-administered medications as allowed by state regulations, such as “reminders.” 12. Offering and assisting with bedpans and urinals. Providing assistance as assigned with light laundry needs, emptying waste baskets and other light homemaker tasks. 13. Performing range of motion and other simple procedures as ordered with appropriate completed/demonstrated skills competency. 14. Providing respite for patient's/family/caregiver when on-site, as appropriate. 15. Keeping patient's living area clean and orderly, as assigned. 16. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. 17. Participates in quality assessment performance improvement teams and activities. 18. Respectful of patient and family/caregiver environment and patient's personal needs. 19. As a mandatory reporter, reports failure to comply with the requirements of chapters 246- 335 WAC and 70.127 RCW to the Washington Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 20. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of RCW 74.34.020 or 26.44.030 to the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. POSITION QUALIFICATIONS 1. Completion of a state approved certified nursing assistant course or equivalent training 2. Certified Nursing Assistant (CNA) in the state of Georgis. 3. Current Healthcare Provider CPR (Cardiopulmonary Resuscitation) certification ((may not be internet based unless the demonstration of skills was hands on and observed by a certified trainer) 4. Current Food Handlers Permit 5. High school diploma or General Equivalency Degree (GED) 6. Ability to read and follow written instructions and document care given. 7. Understands hospice philosophy, comfortable providing specialized care to the terminally ill. 8. JOB DESCRIPTION SUMMARY As an Admissions Nurse, you are responsible as a member of the Interdisciplinary Team for the provision of skilled nursing assessment, planning and care on an on-call basis in order to maximize the comfort and health of patients and families consistent with Hospice policies & procedures. You will collaborate with the patient, caregivers, clinical manager, case managers, and physician(s) to complete the patient care planning at the onset of care. You will provide skilled interventions and instructions to the patient and caregivers to provide optimum symptom control and patient comfort. You will function as a liaison between disciplines during the admission process. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of the nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Provides health care instructions to the patient as appropriate per assessment and plan. 10. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. 11. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. Communication 1. Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. 2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. 3. Communicates with community health related persons to coordinate the care plan. 4. As a mandatory reporter, reports failure to comply with the requirements of the Ohio Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 5. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. 10. Attends interdisciplinary group meetings. Additional Duties 1. Participates in on-call duties as defined by the on-call policy. 2. Ensures that arrangements for equipment and other necessary items and services are available. 3. Supervises ancillary personnel and delegates responsibilities when required. 4. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes. 5.
