We perform in-depth treatment for patients with multiple chronic illnesses including Diabetes, Hypertension, and Lupus. 6A is a fast paced, challenging and rewarding medical unit for new graduate nurses as well as experienced nurses. The staff participates in self scheduling, developing in-services and professional practice committees. Required Qualifications* Current Michigan RN license for all candidates that have passed the NCLEX. GN applicants will be considered for this position. Job Summary Provide safe and effective nursing care as assigned to our congenital and acquired heart disease patients, ranging in age from newborn to adulthood. This position supports the division of Pediatric Cardiology, providing flexible coverage and assistance with the increasing work of the nurses in the department. This work includes: Pre and post procedural care to patients undergoing cardiac catheterization and/or electrophysiology procedure, administering medication and providing vital sign and EKG monitoring for chemical stress testing, and triage support for pediatric cardiology patients. Family centered care is provided in collaboration with all members of the health care team in this highly specialized population. Candidate must be a team player who is highly motivated to provide excellent care in a highly technical environment. Employment Type: Part time Shift: Description: POSITION PURPOSE The Registered Nurse Case Manager assumes overall accountability for patients’ discharge plans while assuming a leadership role in achieving desired clinical, financial and resource utilization outcomes. Encourages residents to socialize and participate in planned, spontaneous, individual or small group person centered programs and to support friendships between residents. Maintains residents' records daily, in a timely manner, and in accordance with company policy and procedures. This includes documentation related to medication distribution as applicable, engagement interests, incidents and observations, reporting any changes in resident's physical condition and/or behavior, visitors, etc. Observes and reports the health and emotional condition of each resident and promptly reports all changes to the supervisor. Only if permitted by state specific regulations, may assist residents with medication as defined in medication procedure.
