Job Title: Care Manager (RN) - Inpatient Department: James | Patient Care Resource Manager Scope of Position/Position Summary The Patient Care Resource Manager, PCRM, is a nurse case manager that facilitates patients through the health care system to include pre-admission through post discharge coordination of care, resources and services utilizing the nursing process. The PCRM monitors and coordinates the patient plan of care to enhance the timeliness of patient care decisions and throughput and to enhance care quality and efficiency and ensure efficient resource utilization . The PCRM identifies and removes barriers to progression of the plan of care for each patient. The PCRM collaborates with physicians, nurses and other health care workers to manage the patient transition to the next level of care. The PCRM is the central information source for third party payers, communicating the plan of care, medical necessity, and gaining approval for post discharge care. The PCRM, through the use of evidence-based criteria, assists the physicians in ensuring the appropriate level of care for each patient. Minimum Education Required Bachelor's Level Degree or equivalent combination of education and experience. Required Qualifications Bachelor’s degree, Masters preferred. RN licensure required; five years recent clinical experience applicable to the population preferred; knowledge of clinical practice guidelines, computer data bases, and payer protocols desired. The team provides patient care and supportive services directed towards achievement of positive patient care outcomes, maximizing the patient’s involvement in their own plan of care, and reducing the length of hospitalization, the staff nurse plays a critical role in staff engagement, the patient experience, quality and safety, and cost-effective care. Position Summary The Staff Nurse is responsible for managing and providing care to the patient and their family, and for directing and supervising nursing care provided by other members of the patient care team. The Staff Nurse is responsible for assessing, planning, implementing, coordinating, delegating, and evaluating the nursing care of patients in alignment with the professional practice model. Minimum Education Required 2-Year College Degree or equivalent combination of education and experience with a Major in nursing Additional Education Desired Bachelor's Degree or equivalent combination of education and experience with a Major in nursing Required Qualifications Degree in Nursing from a CCNE (Commission on Collegiate Nursing Education) or Accreditation Commission for Education in Nursing (ACEN) accredited nursing program required.
