Overview: Diabetes and School Health (DASH) is an enhanced care-coordination program that works to provide the best diabetes care through direct oversight, education and support for patients living with Type I and Type II Diabetes. Services are provided in schools. This position will travel to schools in Columbus and the surrounding area to provide services to students enrolled in the program. Schedule is M-F Day shift. We utilize RN's and LPN's, each taking their own patient assignment with support from our Patient Support Assistants (PSAs).
