Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of "direct" and "indirect" patient care activities. Visits are assigned based on patient and business needs. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed. Location: Moody Outpatient Center 3rd Floor Work Schedule: PRN Days Shift - Monday - Friday 8 a to 5 pm PRIMARY PURPOSE Provides care to assigned patient population in accordance with established protocols and multidisciplinary plan of care utilizing independent, dependent, and interdependent interventions to restore stability, prevent complications, and achieve and maintain optimal patient responses for all patients through patient centered/patient valued care. Assesses patient's discharge readiness and collaborates with other core team members for optimal discharge planning. Evaluates the patient's response to plan of care, and prioritizes and implements appropriate changes as needed. Interprets and organizes sequencing of diagnostic tests and procedures to exercise nursing judgment. Effectively manages triage and practice for patient/family/other internal and external health care professionals to navigate the health care system with nursing supervision.
