SUMMARY: The primary purpose of this job position is to assist in the planning, implementation, and evaluation of recreational, social, intellectual, emotional and spiritual programs, in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. ESSENTIAL DUTIES AND RESPONSIBILITIES: Assist in planning, developing, organizing, implementing & evaluating the activity programs & quality assurance program in this facility for the Activities Department. Observe resident attendance, mood, behavior, and degree of involvement so that the facility activities & resident progress are evaluated, by noting, reporting and charting resident behavior. Assume the authority, responsibility & accountability of Activity Assistant/Aide. Ensure that all charted activity progress notes are informative and descriptive of the services provided and indicate the resident’s response to the service. Develop & maintain a good working rapport with other departments within the facility to assure that daily activities can be performed without interruption. Create & maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the facility. Assist in preparing activity rooms/areas for scheduled activity programs. Recommend to your supervisor the equipment and supply needs of the activity department. Assist in cleaning up activity areas after completing activity functions. Follow the care plan in providing daily activities for resident. Purchase department supplies, clothing and personal items for the residents. Maintain the confidentiality of all resident care information including protected health information & follow facility guidelines in reporting non-compliance with said guidelines & treat residents with dignity at all times. Oversee residents engaged in scheduled activities to promote the physical, social, and mental well-being of residents by applying experience in activities and knowledge of diagnoses and conditions of residents. Assists in-room residents by visiting with them, writing letters, running errands, and perform assistance with crafts, projects, etc., as necessary. The MDS / Care Plan Coordinator is responsible for the accurate and timely completion of all Medicare/Medicaid case-mix documents in order to assure appropriate reimbursement for care and services provided within the Facility. Conducts continual Minimum Data Set (MDS) reviews to assure achievement of optimal allowable Resource Utilization Group (RUG) category. Oversees the overall process and tracking of MDS/Prospective Payment System (PPS) documentation and submission. He/she will integrate nursing, dietary, social recreation, restorative, rehabilitation and physician services to ensure appropriate assessment and reimbursement. ESSENTIAL DUTIES AND RESPONSIBILITIES: Assesses and determines the health status and level of care of all new admissions. Ensures the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, significant change. Communicates level of care for new resident to all disciplines. Coordinates interdisciplinary participation in completing the Minimum Data Set (MDS) for each new admission to facility according to regulatory time frames. Ensures completeness and thoroughness of documentation as mandated by federal, state and medical standards. Maintains an accurate schedule of all MDS assessments to include the proper reference dates throughout the resident’s stay. Responsible for the data entry function to assure accurate data entry and electronic submission of MDS assessments. Verifies electronic submissions of MDS, performs corrections when necessary and maintains appropriate records. Coordinates interdisciplinary participation in completing the MDS for each resident according to regulatory time frames. Ensures completeness and thoroughness of documentation as mandated by federal and state standards. Schedules and conducts resident care conferences in compliance with state and federal regulations and ensures completion of all MDS reviews prior to resident care conference. Assists disciplines in formulating and revising care plans. Ensures that resident’s present/potential problems are identified and prioritized; realistic goals are established and nursing intervention is appropriate. Overview Provides technical support and assistance to nurses and physicians in the care and delivery of pregnant women. Ensures supplies and equipment are readily available, safe and clean, and ensures patient environment is maintained. Organizes equipment and ensures equipment is returned to original state, ready for the next case. Maintains individual physician preference file. Takes inventory of instruments, sutures and patient care supplies. Checks supplies for expiration date and rotates stock. Prepares equipment and supply orders for manager's signature. Assists with completion of birth certificate worksheet, in identified sections, and with transmittal of worksheets to the State. Performs other duties as needed and/or assigned. Overview Join Affiliated Oncologists as a Nurse Practitioner! * We are looking to hire a Nurse Practitioner (NP) for our OLYMPIA FIELDS, IL location. This position is Full-Time and In-Person . This role will specialize in Hematology-Oncology . AO specializes in treating a variety of cancers including lymphoma lung, breast, prostate, gynecologic, colorectal and head and neck cancers. Our physicians and staff are multi-disciplinary team of highly experienced caregivers focused on helping patients and their families receive the best care and support possible. SCOPE: Under supervision or in collaboration with practice physicians, involved in the care of individuals, families and patient groups who are dealing with cancer or other related diagnosis. This care may be in clinic or virtually, via telemedicine. Responsibilities may include duties related to prevention, screening, patient care, management, and research. May serve as direct caregiver, health care consultant, patient educator, administrator, or researcher. Ensures compliance with OSHA and other regulatory standards and requirements. Supports and adheres to the US Oncology Compliance Program, to include the Code of Ethics and Business Standards. Responsibilities ESSENTIAL DUTIES AND RESPONSIBILITIES: - Assesses, plans, implements, and evaluates care for patients. - Initiates and documents appropriate interventions; evaluates and records patient/family response and prescribes therapies as indicated. - Performs procedures with minimal supervision. - Assesses patient/family needs and provides education. - Promotes wellness and recommends community resources to meet patient and family needs. - Often incorporates evidence-based research into clinical practice. - Solid knowledge of reimbursement procedures and documentation requirements. - Completes patient charge capture and medical record documentation in a timely fashion. - Likely to provide case management for patients and their families. - Sometimes serves as a resource for nursing staff by providing information being studied in clinical trials. - May provide palliative care for the patient and their families as needed. - May promote/provide staff education - Responsibilities may include rounding at affiliated hospitals, weekend or after-hour calls - May mentor new advanced practice providers - May precept APP students - Other duties as assigned Qualifications MINIMUM QUALIFICATIONS: Masters of Science in Nursing (MSN) from an accredited nursing school and successful completion of a Nurse Practitioner program. This position typically requires a minimum of 3 years of experience as a practicing Nurse Practitioner. Previous oncology experience strongly preferred. RN state licensure and current registration with the State Board of Nursing in practicing state required. Current state licensure as a Nurse Practitioner required. Current basic life support (BLS)/CPR certification required.
