Overview Case Manager is responsible for chart review to evaluate medical necessity for admission, continued stay, and the appropriate level of care. Monitors the care of assigned patients to assure quality cost effective utilization of resources and optimal outcomes. Responsible for coordinating the patient's plan of care from admission to discharge and or transfer to another level of care. Responsibilities PRINCIPAL DUTIES AND RESPONSIBILITIES(*Essential Functions) Applies to individual case, through chart review, third payer regulations that govern admissions, continued stay, and discharge, i.e. IS/SI. Collects data related to clinical improvement initiatives for the purpose of developing action plans. Conducts concurrent chart reviews to assure complete and accurate medical records documentation. Coordinates the patient's plan of care with all members of the health care team including the patient, caregiver, and or family members and case management. Demonstrates proficiency in patient assessment and documentation. Interviews and assesses patients to determine potential barriers to timely discharge/transfer to another level of care. Provides all payers with accurate, updated information. Obtains covered stay certification and prior authorization as appropriate. Refers cases when appropriate to physician advisor for medical necessity, continued stay, physician education and appropriate level of care. Refers to Social Services when appropriate. Ensures discharge planning to the appropriate level of care, sub-acute, hospital units, and outpatient services. Supports hospital CQI programs by participation on relevant teams. The MDS / Care Plan Coordinator is responsible for the accurate and timely completion of all Medicare/Medicaid case-mix documents in order to assure appropriate reimbursement for care and services provided within the Facility. Conducts continual Minimum Data Set (MDS) reviews to assure achievement of optimal allowable Resource Utilization Group (RUG) category. Oversees the overall process and tracking of MDS/Prospective Payment System (PPS) documentation and submission. He/she will integrate nursing, dietary, social recreation, restorative, rehabilitation and physician services to ensure appropriate assessment and reimbursement. ESSENTIAL DUTIES AND RESPONSIBILITIES: Assesses and determines the health status and level of care of all new admissions. Ensures the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, significant change. Communicates level of care for new resident to all disciplines. Coordinates interdisciplinary participation in completing the Minimum Data Set (MDS) for each new admission to facility according to regulatory time frames. Ensures completeness and thoroughness of documentation as mandated by federal, state and medical standards. Maintains an accurate schedule of all MDS assessments to include the proper reference dates throughout the resident’s stay. Responsible for the data entry function to assure accurate data entry and electronic submission of MDS assessments. Verifies electronic submissions of MDS, performs corrections when necessary and maintains appropriate records. Coordinates interdisciplinary participation in completing the MDS for each resident according to regulatory time frames. Ensures completeness and thoroughness of documentation as mandated by federal and state standards. Schedules and conducts resident care conferences in compliance with state and federal regulations and ensures completion of all MDS reviews prior to resident care conference. Assists disciplines in formulating and revising care plans. Ensures supplies and equipment are readily available, safe and clean, and ensures patient environment is maintained. Sets up and prepares operating rooms and delivery rooms, in accordance with instructions, and physician preference. Ensures rooms are cleaned and turned over timely. Performs scrub duties for obstetrical procedures, to include Cesarean section, hysterectomy, D&C and removal of placenta. Performs instrument and sponge count at beginning and conclusion of procedure. Restocks supplies in OB operating rooms, OB recovery rooms, labor and delivery rooms, patient rooms and nurseries. Organizes equipment and ensures equipment is returned to original state, ready for the next case. Maintains individual physician preference file. Takes inventory of instruments, sutures and patient care supplies. Checks supplies for expiration date and rotates stock. Prepares equipment and supply orders for manager's signature. Overview Supports and guides nursing staff in daily operations. Assumes responsibility for the unit in absence of unit Director. Responsibilities Principal Duties and Responsibilities (Essential Functions) Collects, summarizes and reports all pertinent data to ensure regulatory compliance. This includes all reports that are required for IDOPH reporting Assume patient care duties as needed. Monitors Kronos, develops staff schedules and assignments, editing timecards and assuming other duties of the director as needed. Ensures optimal patient satisfaction by rounding on patients, intervening if necessary to correct situations and monitoring patient satisfaction scores. Developing corrective action plans if results do not meet established goals. Provides clinical oversight to assigned patient population to include staff's competency and assignments and scheduling training and development events. Leads quality improvement initiatives, IE data collection and interpretation as well as design and implementation of action plans as well as staff education regarding quality efforts Leads efforts in RCA reviews or any reviews into patient care issues Performs chart reviews and summaries of cases needed for scheduled morbidity and mortality meetings.
