The Registered Nurse in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. The Registered Nurse coordinates total plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources. All done within a Point of Care setting. Primary Responsibilities Develops, implements, and updates patient plans of care in collaboration with physicians and the interdisciplinary team; ensures home‑health eligibility and completes comprehensive nursing assessments (including OASIS and medication reconciliation). Provides and coordinates high‑quality clinical care; supervises and assigns tasks to home health aides and LPN/LVNs in compliance with state and federal regulations, including required supervisory visits. Accurately documents all patient care per policy and payer requirements; communicates timely with physicians, patients, families, and agency staff to ensure coordinated, effective care. Participates in orientation and precepting of new staff; contributes to performance improvement, survey readiness, patient safety initiatives, and reducing avoidable hospitalizations. Demonstrates professionalism through punctuality, positive attitude, ongoing learning, meeting deadlines, active meeting participation, adherence to agency standards, and participation in on‑call/weekend rotations. As a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will conduct annual in-home health assessments for Medicare Advantage and other plan members. This is a non-prescribing, field-based role focused on improving health outcomes through education, gap closure, and collaboration with primary care providers (PCP). New hires who are eligible but haven't applied yet must do so within 1 month of starting. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. Job Description Summary: The Special Programs Nurse is a licensed clinician who coordinates and provides skilled nursing care in the home or alternate treatment site, ensuring patient safety while providing continuity of care in compliance with the physician’s plan of care. The Special Program Nurse facilitates care for Special Programs in the home and alternate infusion sites. Occasionally cares for patients that do not fall within Special Programs guidelines. The Special Programs Nurse is responsible for the delivery of patient care following the nursing process of assessment, diagnosis, planning, implementation, and evaluation. S/he does so by directing and coordinating all nursing care for patients utilizing established clinical nursing practice standards, ensuring effective and efficient patient care delivery and the achievement of desirable patient outcomes. Strong clinical background with direct knowledge of infusion therapies Proven ability to demonstrate value with initiatives Demonstrated competency in and knowledge of INS standards required for safe delivery of services along with exemplary vascular access skills applicable to Naven Health programs and service needs. Working knowledge of procedures and techniques involved in administering routine and special treatments to patients. Valid Driver’s License and proof of liability insurance with access to a vehicle for business travel. Basic knowledge of computer operating systems and software applications with the ability to apply knowledge and assimilate to the various nursing technology tools utilized by Naven Health to communicate and document care. Able to communicate clearly with patients, caregivers, healthcare team, physicians, administrators and others Working knowledge of all infection control procedures and safety precautions Knowledge of the scope of practice of the registered nurse and ability to apply professional theory, practice and procedure demonstrating appropriate application of the nursing process Physical Demand Requirements Ability to lift or move up to 50 pounds. Fine motor skills and visual acuity are also required (ability to see up close and at a distance, ability to discern color and/or shades, peripheral vision and depth perception required with ability to adjust) Ability to stand for long periods of time, walk, crouch, kneel, turn, pivot, balance, stoop, reach overhead, grasp, push, pull and carry. The Special Program nurse has a high likelihood of occupational exposure to blood, body fluids, and other potentially infectious materials.
