Functioning in lieu of or alongside of a physician, it is essential for an urgent care Advanced Practitioner to have excellent assessment skills in order to recognize more serious conditions and direct the patient to the emergency room for advanced care when necessary. Assist with the admission, discharge, or direct care of residents as needed. QUALIFICATIONS: Current, unencumbered, active license to practice as a Licensed Practical Nurse or Registered Nurse in the state of Connecticut. At least 2 years of experience in MDS preferred Resident Assessment Coordinator (RAC) training completion required within six months of hire; RAC Certification preferred. Intermediate computer skills, including proficiency in Word, Excel, PowerPoint, and electronic medical record systems. Strong organizational, time-management, and independent decision-making skills.
