Our Hospice Nursing Aides may assist patients' family members in caring for their terminally ill relatives by instructing them on how to go about performing basic health-care routines. Nursing Aides consult on a regular basis with their supervising registered nurses in terms of keeping hospice team members apprised of our patients' progression and the families' needs. Qualifications Must meet one of the following requirements. 1) have a minimum of one year full time experience in direct client care in an institutional setting (hospital or nursing facility); 2) have one year experience within the last five years in direct client care in a home health or hospice setting. Must be registered and listed in good standing on the nurse aide registry in the state of employment. Must have basic demonstrated technology skills, including operation of a mobile device. Receive and transcribe written orders from the medical and psychiatric provider Medication Reconciliation process Facilitate effective crisis intervention Assist the RN in consulting and coordinating with healthcare team members to assess, plan, implement, or evaluate patient care plans to include referrals to other levels of care when appropriate Assist the RN in modification of the patient treatment plans as indicated by patients' responses and conditions and collaborate within a multidisciplinary team to achieve best patient outcomes Direct or supervise residential technicians while on shift Provide medical re-education to patients as needed Performs related duties as required. *All agency staff are required to attend all mandatory department/agency meetings and trainings* Schedule: 1st Shift: 7:00 AM - 730 PM Week 1: Tuesday, Wednesday, Saturday Week 2: Sunday, Thursday, Friday Requirements Competencies: Initiative Time Management Decision Making Communication Proficiency Organization Skills Computer competency Education &/Or Experience: Current Connecticut LPN license Basic Life Support (BLS)/ CPR certification EPIC experience preferred, but not required
