How you make a difference The Nurse Practitioner (NP) under the supervision of the Site Medical Director and other site Physicians, offers a comprehensive range of medical services to incarcerated patients in accordance with established policy and state guidelines. They ensure necessary documentation of services is provided to the Site Medical Director or designee to monitor the provision of clinical services. Additionally, they communicate scheduling changes to the Medical Director and H.S.A. and make arrangements for coverage of medical services if unavailable for an extended period of time. Key Responsibilities Conducts routine and emergency medical care for inmates, including chronic care clinics, infirmary rounds, and emergency treatment on-site. Adheres to established policies and procedures for medical care, including documentation in SOAP format, adherence to formulary regimens, and utilizing available in-house resources for treatment. Assumes responsibility and accountability for individual judgments and actions, while also reporting any concerns or questions to the Medical Director or Health Services Administrator. Maintains patient confidentiality and privacy in accordance with accepted practices for incarcerated individuals. Provides input into facility mandatory committees as requested. Additional Details Part-time NP/PA Needed for Two Evening Shifts, Weekly. *New Shift Diff Qualifications & Requirements Education A graduate of an accredited school of nursing An advanced degree in an approved and accredited Nurse Practitioner program. Experience Prior medical/surgical and/or emergency/trauma experience and corrections experience is preferred. Must have the patience and tact to deal with the inmate patient population and ability to work effectively in the corrections environment. Scheduling flexibility is also required to be able to rotate hours and shifts, if needed, and to be called during emergency situations to provide coverage. Licenses/Certifications Must have and maintain current licensure as a Nurse Practitioner within the state of employment. Must have and maintain current CPR certification. Responsibilities POSITION SUMMARY Plan, implement and evaluate nursing care provided, including restorative care for residents in a manner that is reflective the ESCC Core Values. Ensures that the Center and the Department of Nursing policies and procedures are follow in the planning implementation and evaluation of care. Review medication administration records for completeness at end of shift. Give and receives the nursing report and follow through on any special instructions. When in charge makes periodic checks to ensure that prescribed treatments are being properly administered by LPN’s and CNAs and to evaluate the residents physical and emotional status. Participates in the orientation of new staff residents and families to the unit. Carries out restorative and rehabilitative programs including teaching resident to wash/feed/dress self as appropriate. Ensures that the facility policy on the use of restraints is followed and that documentation is timely and accurate. Ensure that the unit is kept clean free from clutter and that the supply room door is kept locked; request the supervisor’s intervention as necessary to achieve this. Follows infection control standards and ensures that assigned personnel do likewise. Keep abreast of new nursing policies and procedures. Keep nurses notes, plan of care and all other documentation pertaining to your assigned resident timely accurate and complete. Carries out end of shift responsibility e.g. supplies are available for next shift’. Unit is clean and tidy; all documentation is complete and accurate. Responsibilities POSITION SUMMARY The Assistant Director of Nursing ensures that standards of professional practice are developed and maintained on a 24-hour basis, 7 days a week to achieve excellence and continuity in resident care on the unit in a manner that is reflective the ESCC Core Values. The Assistant Dir. of Nursing will ensure that nursing care goals are established for each resident and provides guidance for the clinical care team to work toward established goals, ensuring that appropriate nursing care is provided and required documentation is completed. The Assistant Dir. of Nursing will function as an advocate for resident and family, intervene and provide leadership and guidance in crises, and in assuring the resident achieves their highest level of functioning within a least restrictive environment. Responsible for the leadership, direction, supervision, evaluation, hiring and termination of all nursing staff assigned to the neighborhood, the Assistant Dir. of Nursing serves as a support person for members of the staff and acts as a role model to instill in the staff that patient safety, wellbeing and family collaboration are the hallmarks of clinical nursing excellence. The Assistant Director of Nursing will be responsible for working an on-site schedule of a 5 days/week that may include weekends. In collaboration with neighborhood staff, ensure adequate and appropriate par levels of supplies and linen. Perform administrative requirements such as completing forms, reports, etc., and submitting to the Director of Nursing as required. Establish a least restrictive environment where restraints are only used after the care plan team agrees that all other alternatives have been exhausted. Responsible for ensuring regulatory compliance for completion of MDS and Care Plans for each resident according to the pre-determined schedule. Assures that all documentation is completed by established deadlines. Ensures coverage for completion of MDS/Care Plans when assigned staff are unavailable. Coordinate and oversee the Care Plan meetings. Responsible for ensuring completion and follow up of occurrence reports, medication error forms, skin care forms and continually work on reducing occurrences and medication errors through root cause analysis, education and quality improvement processes. Rounds on Neighborhoods for the purpose of obtaining clinical information on residents, to support and educate staff, to assist with family issues and endorses complete information to next shift. Promotes 24 x7 accurate, objective clinical and administrative reports. Responsible for review of EMR status board and other variance reports and follow up with staff on outstanding items. Establish a schedule for and coordinate neighborhood meetings. Assure that information is disseminated to all appropriate parties timely. Review complaints and grievances and make oral/written reports to the Director of Nursing and Vice President of Operations.
