Overview About the Role The Advanced Practice Nurse (APN) is a skilled health provider who through a written collaborative practice agreement with a physician is credentialed through the Medical Staff Office. As a member of the Medical Staff the APN demonstrates standards of professional practice that includes ethical decision making, job activities related to quality assessment/improvement, self- evaluation, leadership, and research. The APN is self-directed and purposeful in seeking necessary knowledge and skills to develop and maintain competency.In this role, with a specialized focus on prenatal patients, the APN will utilize critical judgment in the performance of comprehensive health assessments, differential diagnosis, and the prescribing of pharmacologic and non-pharmacologic treatments in the direct management of patients diagnosed with a variety of Medical/Psychiatric conditions/diseases. The APN practice will promote maternal and fetal wellness through patient education, preventative care and participation in research initiatives. Functioning both autonomously and as part of an interdisciplinary team, the APN serves as a clinical resource and consultant, supporting coordinated care across the prenatal continuum. Responsibilities What You’ll Do Collects comprehensive patient health data/history including past and present medical, surgical, family and social history. Diagnostic tests and procedures relevant to the patient’s status are collected and interpreted. Completes the medication reconciliation process by documenting the current list of medications (including over the counter and nutritional supplements) taken by patient at each visit. Conducts clinical rounds on F-Green, Labor & Delivery, Outpatient Pediatrics and OB/GYN units. Proactively identifies patients with elevated maternal depression or psychiatric risk factors and ensures comprehensive pre- discharge engagement. Provides medical and psychiatric care to patients on the Inpatient unit, in the ED, on medical floors and/or in the Outpatient Clinics. Provides education to patients and families to maximize their utilization of insurance, resources, and physician recommended care practices to improve overall quality of life. Monitors intended and potential side effects of medications and treatments ordered and will offer appropriate, accepted interventions. Provides monitoring and continuity of care between departments/services; collaborates with Clinical Care Coordinators, Patient Navigators, and medical staff to coordinate continuity of care. Responsibilities The primary purpose of the Assistant Director of Nursing UH (Unit Based) position is to operationalize and transition changes in the care delivery system of a unit or set of units through the skillful directing, coaching, support, and delegation to staff in the provision of clinically safe, fiscally sound, compassionate care and services to achieve safety, patient/family goals and patients/family satisfaction. The Assistant Director has (24) hour responsibility for the unit’s service area and develops a dynamically functioning interdisciplinary team. She/he sustains the daily operational functioning of the units/services area and makes immediate modifications to prevent impediments, errors, and delays in goals achievement. This role is also accountable for the execution of the units/services area Performance Improvement Plan. She/he engenders confidence, trust and fosters collaborative/collegial relationships that generate staff achievement and pride. The Assistant Director facilitates an atmosphere of interactive management and the development of collegial relationships among the nursing personnel and other disciplines. The Assistant Director facilitates change through education, demonstrating skill in implementing practices changes. She/he collaborates with the Department of Education and Professional Development and assists in presentations, orientations, and other house-wide education/training Qualifications Licensed as a Registered Nurse in the State of New Jersey. Bachelor’s Degree in Nursing is required. Master’s Degree in a related field is preferred. One (1) year minimum of nursing managerial experience preferred. Minimum of three (3) years clinical care delivery experience. BLS (CPR) is required.

How you make a difference The Nurse Practitioner (NP) under the supervision of the Site Medical Director and other site Physicians, offers a comprehensive range of medical services to incarcerated patients in accordance with established policy and state guidelines. They ensure necessary documentation of services is provided to the Site Medical Director or designee to monitor the provision of clinical services. Additionally, they communicate scheduling changes to the Medical Director and H.S.A. and make arrangements for coverage of medical services if unavailable for an extended period of time. Key Responsibilities Provides medical care to inmates, including conducting exams, chronic care clinics, and infirmary rounds. Maintains accurate and legible progress note documentation in SOAP format and adheres to formulary guidelines. Responds to emergencies and makes logical decisions regarding non-formulary drugs or off-site referrals. Supports existing policies and procedures for admission, transfer, and specialty consultant referrals. Maintains confidentiality and provides input into facility committees as requested. Qualifications & Requirements Education A graduate of an accredited school of nursing An advanced degree in an approved and accredited Nurse Practitioner program. Experience Prior medical/surgical and/or emergency/trauma experience and corrections experience is preferred. Must have the patience and tact to deal with the inmate patient population and ability to work effectively in the corrections environment. Scheduling flexibility is also required to be able to rotate hours and shifts, if needed, and to be called during emergency situations to provide coverage. Licenses/Certifications Must have and maintain current licensure as a Nurse Practitioner within the state of employment. Must have and maintain current CPR certification.
