Assess each patient and perform triage duties as assigned. Formulate and carry out a goal directed plan of care which based on determined nursing diagnosis and patient outcomes and which is prioritized according to patient needs and available resources including time, personnel, equipment, and supplies. Evaluate effectiveness of self-care given by all health team members, and contribution of systems, environment, and instrumentation in progressing patient toward outcomes. Provide treatment and discharge instructions upon patient release. Perform assessment/data collection in an ongoing and systematic manner, focusing on physiological, psychological, and cognitive status. Provide a safe and clean environment for each patient. Identify patient/significant other learning needs and implement appropriate measures to meet identified needs. Assist in planning, provide clinical direction, and provide instruction to LPNs/LVNs, nursing assistants and ancillary personnel. Communicate with patients utilizing Secure Messaging to improve communication and facilitate care through non-traditional means The HCWs shall complete annual training requirements including but not limited to the Command Respirator Fitness Testing Program. Complete unit and required command competencies including but not limited to: Intravenous (IV) access/IV therapy, Medication Administration, environment of care, unit specific ER nursing and equipment competencies following NHCP Policies and protocols during shadow orientation shifts. Must be completed prior to working independently. Complete all Unit Specific Emergency Department and required command competencies including but not limited to Restraints, Massive Transfusion Protocol, administration of blood products, triage and IV Conscious Sedation following NHCP Policies and protocols within 90 days of assignment at NHCP. Receive phone calls and/or patients presenting in the Emergency Room Department and triage patients to appropriate level of care within the military treatment facility. Qualifications Minimum Qualifications : • Education: Possess a Baccalaureate Degree in Nursing, a Graduate Diploma in Nursing, or an Associate Degree in Nursing, from an accredited agency such as the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE). • Experience: Possess a minimum of three years-experience within the last four years as an Emergency Room RN OR if there was a break in employment, the HCW shall have a minimum of three years extensive previous experience as an Emergency Room RN within the last six years OR HCWs with any prior military experience as an Emergency Room Registered Nurse must have at least one of experience. • Licensure: Current, full, active and unrestricted license as a Registered Nurse * Security : Must possess ability to pass a Government background check/security clearance. * Life Support Certification: Possess a current AHA OR ARC BLS Healthcare Provider certification, ACLS & PALS. As part of the MHP team, the HCWs shall raise awareness about behavioral health issues and care facilitator services in MHP practice area to promote easy referral of patients. Inform the Clinic Manager of administrative/clinical problems as they arise. Participate in all accreditation and Medical Home Port recognition processes (JC, IG, etc.). Engage other MHP team members to assist in improving documentation of the episode of care using MHP Clinic-approved templates as the initial template (i.e., Tri- Service Workflow/COMPASS). Coordinate schedule and non-availability (i.e., leave, training, etc.) with other BHIPMHP team members to ensure adequate access to care. Participate in MHP team huddles.
