Overview RN / Registered Nurse, Home Health Location: San Marcos Office Position: RN Case Manager, Home Health Position Type: Full-Time Remote/Virtual Position: No Coverage Area: San Marcos and surrounding areas Find Your Passion and Purpose as an Registered Nurse, Home Health Case Manager Salary: $108,058.78 to $132,071.84 Annually Schedule: Monday to Friday | 8am to 5 pm Offer Based on Years of Experience What You Need to Know An RN Case Manager is responsible for managing the care plans of the patients from admission through discharge and for maintaining the delivery of quality patient care. The position combines the activities of direct and indirect patient care and directs staff nurses, LVNs, CHHAs in the delivery of the individual patient’s plan of care as well as identifying interdisciplinary needs and coordination of the allied health clinicians. Be the Best RN Case Manager You Can Be If you meet these qualifications, we would love to meet you: Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation. A minimum of one year of experience as an RN. Current CPR Certification required. Responsibilities: As a RN Case Manager, you will: Assessment of the status of nursing needs of assigned patients and overall management of the patient’s care plan. Completes initial patient assessment upon admission including appropriate patient history. Selects appropriate nursing diagnosis. Assess all medicines to identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, and contraindicated medications. Establishes realistic measurable, observable goals consistent with the patient’s diagnosis. Informs the physician of the results of the assessment and recommended clinical interventions. Implements appropriate nursing interventions consistent with the patient's diagnosis and established goals and within the scope of the Nurse Practice Act. Re-evaluates the patients nursing, and other clinical needs as required. Documents and submits the assessment, progress and discharge notes and other required paperwork in a timely fashion as required by the agency. Informs the physician and other personnel of changes in the patient's condition and needs.
