Overview Directs, coordinates, manages, and oversees the activities of the Medical Care at Home (MCAH) clinical staff to ensure the highest level of patient-centered care, including quality and financial outcomes. Collaborates with MCAH leadership and other VNS Health departments and stakeholders in the development, design, and implementation of care management program for the Practice. Ensures evidence-based standards of care are integrated into practice. Provides medication management and assessment on an ongoing basis. Provides clinical supervision and oversight for field staff, including onsite observation of direct patient care to ensure quality of services.
