Overview Endocrinology Clinic 40 hours per week This position is Monday through Friday with the opportunity to work remotely 1-2 days/week after an initial orientation period. Overview Prep & Recovery, Outpatient Surgery Center (stand-alone facility on campus) Full-time, Monday – Friday; 4x10 hour shifts, 6:00am – 6:30pm Admission & Recovery Area Nurse Post Anesthesia Care Unit Registered Nurse - Outpatient Surgery Center (OSC PACU RN) provides care to patients in accordance with ASPAN (Association Society of PeriAnesthesia Nurses) guidelines. PACU RNs provide care and support to patients before, during, and after surgery. The Outpatient Surgery Center (OSC) At Dartmouth Hitchcock is a free standing center located on the DHMC Lebanon campus. The OSC operates eight operating room suites and is dedicated to providing care for adult and pediatric patients requiring same-day surgical procedures. Overview Our Academic Medical Center has expanded to meet the inpatient psychiatric care needs of adolescent patients in our rural New England region. Our six-bed Adolescent Medical Psychiatric Care unit provides specialized, patient-and-family centered care for adolescents ages 12-17. The interdisciplinary team supports patients through comprehensive assessment, individualized treatment planning, psychiatric care, and evidence-based therapeutic approaches, including cognitive behavioral therapy, dialectical behavior therapy, and family therapy. Overview The Pre-Admission Testing Registered Nurse is responsible for ensuring preoperative optimization including patient education for those patients undergoing surgical and endoscopic procedures. The nurse works closely with key stakeholders in coordinating the patients’ perioperative needs. Responsibilities Assessment and Review of Records Monitor patient medical records for preoperative testing results and exercise professional nursing judgement to identify and address barriers to optimization for surgical, diagnostic, invasive, noninvasive and other special procedures. Performs accurate and comprehensive review of clinical factors, past medical history and relevant information. Use effective interviewing skills to perform patient health interviews with surgical and endoscopy patient by collecting and analyzing clinical and psychosocial data, identifying problems and health patterns/trends which may directly affect optimization for surgical/endoscopy procedures. Promote and maintain patient safety by assessing clinical and psychological needs in preparation for the day of surgery/procedure. Clinical Knowledge Utilize clinical knowledge to ensure optimization of patients scheduled for surgery prior to their admission date to prevent surgery cancellations and delays. Utilizes clinical knowledge and established perioperative protocols (surgical and anesthetic) to provide patient with individualized preoperative education. Demonstrates the ability to identify areas of knowledge deficit, and provides additional needed information to the patient. Possess adequate understanding of operative procedure, required patient preparations, and expected post-operative course. Care Coordination/Collaboration/Communication Bring pertinent information obtain during chart review and patient interview to the care team. Alerts the team to pertinent information that would affect patients’ perioperative risk and areas that are a risk for a safe discharge. Demonstrates ability to work both independently and as an effective member of the health care team. Take an active role in strengthening the climate for acceptance of positive change and initiatives that improve patient care outcomes. Provides assistance to the Post Anesthesia Care Unit (PACU) as case load indicates to the level of training. Participates in quality assessment and improvement plan as outlined by the department. Communicates with appropriate providers when indicated to ensure patients are optimized for surgical procedure and anesthesia administration. Communicates outstanding preoperative clearance requirements with appropriate members of the surgical team. Complies with hospital and departmental policies, procedures, regulations and standards. Overview Our Behavioral Health Emergency Department unit consists of a 6 beds supporting high risk mental health patients during a critical psychiatric event. Overview Integrate collaborative care planning for individual patients and implement nursing standards in the processes of care. Responsibilities Utilizes the nursing process in caring for all patients, to include: assessing symptoms, planning interventions, implementing treatment plans, evaluating responses to interventions, and revising plan based on overall effectiveness. Overview As part of the New London Hospital Medical Group, ensures safe and effective patient transitions across the patientcare continuum. As a patient educator, advocate, and patient empowerment facilitator, this position providesinformation to the patient and/or caregiver resulting in effective care transitions, improved self-management skills,and enhanced communications between the patient and the healthcare team. Responsibilities Transitional Care Responsibilities Attends discharge planning meetings. Reviews the patient’s medical record and gathers clinical information from the current care delivery team and patient. Provides information to patients and caregivers about NLH post acute discharge plans. Collaborates with all members of the interdisciplinary team to develop a safe transitional plan to the next level of care. Ensures all clinical and personal transitional goals are met throughout. Advocates on behalf of the patient and the healthcare provider for needed patient resources and services. Provides written communication to the receiving clinician as the patient transitions from one level of care to the next. Contacts the patient’s PCP with each transition of care, providing a summary of the patient’s clinical status. Collaborates with the PCP to provide direction to enhance the effectiveness of the healthcare delivery team, including transitions across physical settings and to monitor and manage changes in health status. Maintains excellent documentation of patient encounter records. Enters specified data elements into electronic dashboard with each patient transition or discharge. Develops and maintains plan of care for NLH-based patients.
