PURPOSE AND SCOPE: The professional registered nurse Outpatient RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. This may include delegation of appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs and Patient Care Technicians. As a member of the End Stage Kidney Disease (ESKD) health care team, this position participates in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations. PRINCIPAL DUTIES AND RESPONSIBILITIES: All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations. · Performs all essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor or in collaboration with other Registered Nurses. · Performs ongoing, systematic collection and analysis of patient data pre – during – post hemodialysis treatment for assigned patients and documents in the patient medical record, makes adjustments or modifications to treatment plan as indicated and notifies Team Leader, Charge Nurse, Supervisor or Physician as needed. · Assesses, collaborates, and documents patient/family’s basic learning needs to provide initial and ongoing education to patients and family. · Directs and provides, in collaboration with direct and ancillary patient care staff, all aspects of the daily provision of safe and effective delivery of hemodialysis therapy to assigned patients. · Administers medications as prescribed or in accordance with approved algorithm(s), and documents appropriate medical justification and effectiveness. · Initiates or assists with emergency response measures. · Serves as a resource for health care team, participates in staff training and orientation of new staff as assigned. · Ensures correct laboratory collection, processing and shipping procedures are performed and reschedules missed or insufficient laboratory collections. · Identifies expected outcomes, documents and updates the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team. · Ensures patient awareness related to transplant and treatment modality options. · Required to complete CAP requirements to advance. · Performs all other duties as assigned by Supervisor. SUPERVISION: Assigned oversight of Patient Care Technicians/LPNs/LVNs/RNs as a Team Leader or designated Nurse in Charge, after meeting all the following: · Successful completion of all FKC education and training requirements for new employees. · Must have a minimum of 9 months experience as a RN. · Must have a minimum of 3 months experience in chronic/acute hemodialysis as a RN. EDUCATION and LICENSURE: · Graduate of an accredited School of Nursing. · Current appropriate state licensure. · Current or successful completion of CPR BLS Certification. · Must meet the practice requirements in the state in which he or she is employed. EXPERIENCE AND REQUIRED SKILLS: · Entry level for RNs with less than 2 years of Nephrology Nursing experience as a Registered Nurse. · Chronic/acute hemodialysis experience (preferred). The rate of pay for this position will depend on the successful candidate’s work location and qualifications, including relevant education, work experience, skills, and competencies. The Clinic Registered Nurse (RN) functions within the nursing scope of practice under the direction and supervision of the Provider (physician, PA or ARNP) and is responsible for assisting in providing care and implementing delegated activities of care. Interviews patient, documents chief complaint, measures vital signs and accurately records information in Electronic Medical Records (EMR) as appropriate. Establishes priorities in organizing daily activities, effectively manages patient flow and anticipates special equipment, solutions and miscellaneous supplies needed for various procedures. Triages patients to the level of care necessary to provide quality patient care with the best possible results based on triage protocols and consultation with Providers. Assist/Initiate with BLS measures. Instructs patient in collection of samples and tests. Consistently demonstrates correct handling of procedure specimens per Provider orders, including appropriate identification, labeling, dating and disposition of the specimen. Communicates test results and instructions to patients per practice protocol. Acts as a content expert and clinical resource by providing staff and patient education. Educates patient and care givers about diagnostic procedures, medications, chronic disease management and treatment instructions (e. diabetes, AIDS, immunizations). Carries out orders as approved by the supervising Providers. Assists Provider by preparing the patients and equipment for examinations and procedures. Assists the Provider during procedures and treatments. Assesses surgical and acute wounds to assist the Provider with treatment. Understands the proper application of splints, slings, cast boots, braces, etc. Measures for knee braces and other appliances. Starts and monitors IV as directed. Ensures proper inventorying and ordering of medical supplies and maintenance of emergency equipment. Assists with medication management for patients. Accurately and promptly requests prescription authorizations and orders refills on medications as authorized by Provider and/or practice policy. Understands surgical inpatient procedures and care. Performs the following non-face to face services: Referral management Communication of test results Development and updating of Care Plans Medication management Assistance with connections and communication with community resources Patient education and communication Assists with documentation in documentation platforms as required in addition to appropriate documentation within the EMR. Acts as a resource for patient care and ensure that patients are scheduled with the Provider that is best suited to meet their healthcare needs. Maintains confidentiality of all information related to patients, medical staff, and employees as appropriate. Understands and has knowledge of diagnoses, billing, and procedures. This position may be primarily assigned to specific medical specialty department but may be asked to work in other departments including Pediatrics, Orthopedics, Family Medicine, OB/Women’s Health, Urgent Care, Podiatry, and other departments as requested. EDUCATION & EXPERIENCE Education: Graduate of an accredited registered nurse program. BSN preferred. License: Current Washington state Registered Nurse license. Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of "direct" and "indirect" patient care activities. Visits are assigned based on patient and business needs. Summary The Infusion Therapy RN is responsible for the comprehensive holistic care of oncology and non-oncology patients being infused with chemotherapy, biologic therapy agents, blood products, fluids, and other medications. The nurse provides care coordination and direct patient care; consults with physicians and colleagues; and educates patient/family on symptom management and new therapies, for oncology and non-oncology patients, treated in the infusion department. This position provides longitudinal comprehensive nursing care to oncology/hematology patients and to non-oncology patients requiring infusion therapy and complex cancer treatments. This position works in collaboration with all members of the healthcare team to achieve the best possible outcomes for the patient. Position Reports To : Clinical Practice Manager Essential Functions Demonstrates a high level of understanding of oncology illnesses and treatment modalities. Demonstrates the ability to assess baseline physical, emotional and social needs. Provides clinical and technical oversight to patients requiring infused or injected chemotherapy, biologic therapy, blood products, and other agents: Performs a nursing assessment prior to and during each infusion or treatment per policy and procedure of the medical center. Has knowledge of agents infused including the potential drug toxicities, side effects, proper methods of administration, and interventions necessary for the safety of the patient. Safely prepares infusion and pump set-up for each drug that is infused. Follows parental administration policy and procedure of the medical center for the medications given. Provides comprehensive care to patients as part of the overall care team.
