Title: Nurse Practitioner, Advanced Practice Provider Company: Oak Street Health Role Description: The purpose of a Nurse Practitioner at Oak Street Health is to provide effective and equitable value-based primary care to adults on medicare to keep them happy, healthy, and out of the hospital. Nurse Practitioners see patients independently and collaborate with physicians, depending on the patient’s specific situation. Nurse Practitioners provide longitudinal care and build meaningful patient relationships; this creates trust and helps our patients understand and feel connected to their care plan. Nurse Practitioners practice in our outpatient practices on a collaborative care team composed of a physician, nurse practitioner or physician assistant, medical assistants, a dedicated medical scribe, and support from registered nurses and care managers. We partner with a network of elite specialists and hospitals for specialty and acute care. As such, our healthcare providers can focus on care within the clinic: primary care, care coordination, and population health. Core Responsibilities: Provision of exceptional primary care. Conduct office visits for routine and acute issues. Administer Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation. Care coordination with other providers, specialists, testing facilities, and agencies. Population health leadership, in coordination with the Care Team (e.g., making sure all eligible females get their evidence-based breast cancer screening every 2 years) Assisting the care team with phone triage and outreach. Educating patients on their health conditions, care plans, and treatments. Participating in Oak Street Health promotional activities. Conducting home visits as needed. Other duties, as assigned. Essential Functions: Ensure clinical services are delivered in accordance with state regulations, accepted clinical standards, and community policies and procedures Assist in coordinating and managing nursing and personal care services for residents Provide nursing services, wellness counseling, health promotion, and disease prevention support Assist residents with activities of daily living, including bathing, dressing, eating, toileting, hygiene, and mobility Oversee and verify resident medication regimens in collaboration with physicians and healthcare providers Conduct resident assessments, participate in pre-move-in evaluations, and assist with service plan development and reviews Coordinate resident care with families, physicians, and outside healthcare agencies as needed Lead and participate in Service Plan Reviews with residents, families, and interdisciplinary team members Maintain knowledge of assisted living and memory care regulations to ensure ongoing regulatory compliance Monitor infection control, immunization compliance, and infectious disease management practices Conduct routine audits of resident care systems, documentation, delegation practices, and quality assurance tools Support move-in, transition, and discharge processes for residents Assist with staff orientation, ongoing education, training, and competency development Supervise and support resident care associates in the delivery of quality care and services Review resident care documentation to ensure accuracy, compliance, and timely communication Provide delegation in accordance with state regulations and community policies Assist with resident care staffing coordination and scheduling Serve as an after-hours/on-call resource for resident care needs, including onsite response when necessary Provide regular updates and reports to community leadership regarding resident care services and concerns Participate in additional resident care and community responsibilities as assigned by leadership Education/Experience/Licensure/Certification Must be a Licensed Practical Nurse or Registered Nurse, licensed to practice in the state. Experience in Assisted Living, Home Health, Skilled Nursing and/or retirement center. Experience and passion in working with seniors. Experience in supervisory role and able to lead. Able to perform tasks which may be physically demanding such as pushing, bending and lifting up to 60lbs.
