Location SAINT LOUIS, MO 63110 Scheduled Hours 40 Position Summary The Nurse Practitioner or Physician Assistant will join a collaborative outpatient cardiology team focused on providing exceptional care to adult general cardiology patients at the CAM at Barnes Jewish Hospital. Working alongside physicians, advanced practice providers, and registered nurses, the APP will primarily manage follow-up visits for established patients, while occasionally seeing appropriately triaged new patients. This role helps improve patient access by allowing physicians to focus on additional new patient appointments. Schedule & Flexibility • 8-9 hour workdays. • Seven half-day clinic sessions per week. • Onsite clinic responsibilities with flexibility to complete administrative work remotely. This position offers the opportunity to build long-term patient relationships while delivering high-quality cardiovascular care in a supportive, team-based environment. Job Description Primary Duties & Responsibilities: Provides direct patient care to include obtaining patient history, performing patient assessment via observation, interview and examination; orders, performs and interprets diagnostic studies; performs preventative health assessments, screenings, immunizations and care; provides direct treatment and management of acute and chronic disease conditions; develops and implements patient-centered plan of care in collaboration with physicians, patients, consultants, nurses, ancillary care providers and others as appropriate to the patient's condition. May bill for services covered by collaborative practice agreement. Provides counseling and education of patients and their families/caregivers concerning disease management, complication prevention, disease prevention, and quality of life improvement measures, and outcome management activities. Monitors/ensures accuracy of patient medical records; maintains all required reports, records, statistics, etc. Maintains a safe, comfortable and therapeutic environment for patients/families in accordance with established clinic/hospital standards. Manages health conditions via prescriptive authority or referral to other healthcare providers or community resources; may perform diagnostic or therapeutic interventions consistent with certification and state regulations. Champions the safety and quality of care for patients using ITB therapy and serves as an expert in the care and management of ITB pumps. Helps develop a detailed educational program for the patients and lead the quality and safety initiative for the program. Serves as clinical expert regarding rehabilitation nursing while demonstrating a high level of autonomy and expert skill in complex clinical nursing situations. Provides management services for clinical research study projects; monitors participating subjects, including medication administration and family/patient education. Works in accordance with established departmental policies and procedures, objectives, quality assurance programs, and safety, environmental and infection control standards. Regularly participates in constant quality improvement and process improvement activities centered on the goal of providing exceptional, evidence-based patient care. Enhances professional growth and development through educational programs, current literature, in-service meetings and workshops. Memorial Hospital Shiloh , a 94-bed, all-private suite hospital was recognized with Magnet status in 2018 and provides emergency care, labor & delivery, nursery, medical and surgical services plus critical care. Two Northeast cares for patients pre and post cardiac catheterizations, implantable cardiac defibrillators, pacemaker insertions, percutaneous transluminal coronary intervention with and without sheaths, post thoracotomy, respiratory problems, and renal patients. Preferred Qualifications Role Purpose Provides direct patient care activities including assessment, diagnosis, planning implementation, and evaluation within the guidelines of the standards of nursing care. Responsibilities Promotes patient and family centered care in a healing environment. Educates patients and their families on how to manage their illness or injury, including post treatment home care needs and medication administration. Participates in activities that promote patient safety, quality and regulatory compliance. Participates in professional development. Develops, implements, and documents individual plans of care with defined goals in collaboration with other members of the interprofessional team and patient, family or caregiver in accordance with the established guidelines and standards of nursing care. Proactively plans and ensures communication of the plan of care across the continuum of care. Uses critical nursing skills to assess and evaluate physical, psychosocial, and emotional needs according to standards of care. Assess patient preferences and barriers to involvement in care, including their values, emotional, spiritual, cultural, and population-specific needs.
