Responsibilities Maintains current assessment and documentation of resident health status: Performs initial and on-going assessments per the applicable Minnesota Assisted Living regulations and Nurse Practice Act. Ensures assessments are completed every 90 days per regulatory requirements. Works with resident and/or responsible party through collaboration, education, and timely communication to ensure appropriate service and placement. Service Agreements: assure that resident has an up to date service agreement at time of admission, with any changes in services and annually. Develops, maintains, and implements individual emergency plans based on health status and Advance Care Planning documents, including POLST.
