Duties and Responsibilities: Assist with daily living skills (e.g., lifting, transferring, toileting, personal hygiene, laundry, meal preparation). Promote appropriate community integration and social interaction (e.g. visiting family, friends, church/temple, etc). Implement outcomes developed through a Support Plan. Complete paperwork as assigned including documentation (e.g., incident reports, outcomes, log entries). Administer and record medications pursuant to State and Agency requirements, where applicable. With valid APD training Medication Administration Fl Code 65 G-7. Accompany residents on medical appointments when needed. Maintain living environment in accordance with D.D.A. regulations and health department standards. Provide Transportation to our consumer as needed. Make oneself available in emergency situations as the need arises. Work cooperatively with professional and paraprofessional staff in the interest of the consumer. Communicate with and update the Supported Living Coah on a regular basis, if applicable. Assist the consumer with recreational, cultural and vacation plans of his/her choice. Provide independent and functional living instructions to the consumers. Complete all state required trainings. Assist with shopping (e.g., food, clothes). Education and Experience : High School Diploma or G.E.D. Required, copy. Must have valid driver’s license and a clean driving record. Must be able to work independently. Must have 1 year of experience with Disabled Children/Adult, Nursing Home or Home Care.
