Job Description The Clinical Registered Nurse (RN) II is responsible for managing individualized patient care by promoting and restoring patients' health in accordance with medical and nursing plans of care and established policies and procedures. The Clinical RN II collaborates with physicians and multidisciplinary team members in the development, execution, and evaluation of the multidisciplinary plan of care. Job Description The PACE Center Manager RN has full responsibility for all disciplines and services provided through Centra PACE Center and administrative tasks related to direct operation of the program. This role provides clinical oversight to the Lynchburg PACE program, Bedford Alternative Care site, and Amherst Alternative Care site. Responsibilities Essential Duties and Responsibilities: Manages the daily operations of the Lynchburg PACE site under the guidance of the Site Director Coordinates care planning and implementation, preparation for interdisciplinary conferences, and meetings with family and participants.
