Coordinates the effective and efficient delivery of patient care services through the application of care management principles. Identifies clinical priorities and initiates action to improve patient outcomes. Demonstrates ability to function in a professional practice model through active participation in shared governance, promotion of a healthy work environment and acceptance of personal responsibility for professional growth. PRN Status Clause: PRN / “as-needed” means you are not guaranteed a fixed schedule or hours. You work when they have needs in the schedule. You must be available to work at least 12 hours every two weeks (bi-weekly minimum), as specified in your PRN Commitment Agreement. Your actual hours will depend on department needs, not a set schedule. You may be asked to pick up extra shifts to cover gaps (like call-outs or shortages). Your total hours can go up or down—some weeks you might work more, some less. Our Certified Nursing Assistant – CNA (State Tested Nursing Assistant-STNA in Ohio) work with community management to provide seniors with personalized care, and give resident status updates at the beginning and end of each shift. Nursing assistants check in with residents, assist with dining and personal care needs, and perform vital sign checks and clinical procedures according to community policy. Provide assistance with activities of daily living (showers, bathroom assistance, dressing/grooming) Successful completion of State CNA/STNA course is required. Must maintain certification.
