Full time/72 hours per pay Days/7:00am-7:30pm The Step-down Unit is a 16-bed inpatient nursing care unit that serves a variety of telemetry monitored patients. The unit specializes in the treatment of a multitude of conditions and diagnoses including but not limited to chest pain, MI, CHF, cardiac arrhythmias, post procedures (i.e PCI, ICD, permanent pacemakers), trauma, stable DKA, and stroke/TIA. The Clinical Nurse is a registered nurse (RN) who partners with and advocates for patients/families/significant others to provide physical, emotional and spiritual support which impacts patient outcomes in a safe environment. The RN functions in a versatile role that demonstrates professionalism and promotes excellence and autonomy in the practice of Nursing. The RN utilizes the nursing process in conjunction with evidence-based principles to care for a diverse caseload of patients within the department scope of service. The RN serves as guide, healer, collaborator, teacher, leader and sentry. The RN collaborates with and guides care delivered by the inter-professional team. JOB DESCRIPTION SUMMARY The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Drug Administration Functions • Prepare and administer medications as ordered by the physician. • Verify the identity of the resident before administering the medication/treatment. • Ensure that prescribed medication for one resident is not administered to another. • Ensure that an adequate supply of floor stock medications, supplies, and equipment is on hand to meet the nursing needs of the residents. Checks table(s) for complete readiness for each procedure. Ensures all items needed for the case are present: Obtains physician’s special instruments and sutures prior to start of the case.
