The Charge Nurse - LPN supervises the day-to-day nursing activities of the Certified Nursing Assistants (CNA) in accordance with company standards and federal, state, and local standards, guidelines, and regulations to ensure that the highest degree of quality care is maintained at all times. Essential Activities and Tasks Operations Management and Administration - 70% Directs the day-to-day functions of the nursing activities in accordance with current rules, regulations, and guidelines that govern the facility, including the making of work assignments to Nursing Assistants. Assists in implementing methods for coordinating nursing services to ensure continuity of residents care. Administers medications and treatments in accordance with physician orders and nursing standards of practice. Represents the nursing department during Interdisciplinary Care Plan meetings. Is able to answer questions related to the plan of care and delivery of services. Participates in various committee meetings. Makes work assignments and meets with Nursing Assistants regularly to assist in identifying and correcting problem areas, and the improvement of care and services. Provides direct nursing care as needed. Consults with the resident’s physicians in planning resident care, treatment, rehabilitation, etc. as needed. Notifies residents’ physicians and families when there is a change in the resident’s condition. Authorizes and monitors the use of restraints when necessary and in accordance with the facility policies and procedures. Admits, transfers and discharges patients as necessary. Works with Unit Manager to help reduce hospital readmissions. Attends all-employee and departmental meetings and in-service training as scheduled or as directed. Quality, Compliance, and Risk Management - 20% Ensures that nursing procedures are followed by nursing service personnel, and that they reflect the day-to-day nursing care procedures used by the facility. Participates in the maintenance and implementation of the facility’s quality assurance program. Ensures the procedures for the administration, storage and control of medications and supplies are being followed by nursing staff. Ensures that resident rooms, treatment areas, work areas and equipment are maintained in a clean, sanitary, and safe manner. People Management and Development - 10% Supervises CNAs. Fosters a positive work environment that attracts, retains, and motivates employees. Interviews, selects, and orients new direct reports. Defines, establishes, and promotes performance expectations so direct reports understand and demonstrate excellence through high quality and consistent performance. Evaluates job performance of direct reports in accordance with the company policy and procedure and provides developmental guidance to staff for improved performance and growth. Monitors accountability with organization requirements prescribed by the Employee Handbook. Documents and engages employees who are routinely not observing these requirements in an effort to achieve compliance. Works with Human Resources when circumstances arise that necessitate further documentation and intervention. Participates in the development, planning, scheduling, conducting, and monitoring of in-service training programs, on-the-job training, and orientation programs for departmental staff. Ensures that there is a reasonable and sufficient plan for staffing should a direct report be absent or a direct reporting position be vacant. Responds timely and thoughtfully to questions, requests, and work product from departmental staff that has been provided for review and approval. All other duties as assigned. Position Summary: The Assistant Wellness Director is responsible for the overall direction of clinical resident care services by delivering a 1440 experience, optimizing performance and leading employee connections within our community. Required Experience for Assistant Wellness Director: Licensed Practical Nurse (LPN) or Registered Nurse (RN) registered within the State preferred. Assisted Living/Memory Care experience preferred. Prior supervisory experience required. Earned a degree from an educational institution in health care, mental health or social services profession or be a licensed Health Facility Administrator. Minimum of 1 year of experience with dementia or Alzheimer’s residents, or both, within the past 5 years. Minimum of 12 hours of dementia-specific training within the last 3 months. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to communicate effectively before groups of customers or employees of organization. Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Proven organization and communication skills. Ability to work successfully through complex situations (ability to problem solve). Accountabilities for Assistant Wellness Director: Lead, manage, and overall accountability for your team and their performance. Revenue, Evaluations & Care Conferences for IL/AL: Responsible for ensuring right care, at the right price with the right staff. Employee Connection: Lead to Employee First by setting clear expectations, giving and receiving regular and honest feedback, and growing and recognizing your staff. Resident & Family Care: Oversee and provide care for our residents and communicate clearly and regularly with their families where appropriate. 1440 Care Standard Accountability: Understanding and leading to our 1440 care standards. Other Key Responsibilities for Assistant Wellness Director: Perform admission and ongoing evaluations of residents quarterly, upon return from Hospital or skilled environment, or change of condition (initiate care conference) and use this information to set measurable goals and outcomes for residents that meet the physical, mental, and psychosocial needs. Obtain written orders for any necessary changes to care plan. Completes 6 hours of dementia-specific training annually to meet the needs or preferences, of cognitively impaired residents and understands current standards of care. Verification of MARS/TARS and communicate with pharmacy to assure accuracy of medication & treatment administration. Reviews occurrence reports submitted by staff members to determine if an assessment/care conference needs to be performed on an existing resident & address any personnel issues which are identified. Schedule educational sessions relating to health for all residents. Communication of all emergency policies and provision of updated information to staff. Work toward continual improvement of the overall organization. Available to work a flexible schedule including on call, weekends, and holidays when necessary. Perform other duties as assigned. Memory Care Coordinator Responsibilities include but are not limited to: · Plan and coordinate a therapeutic program which meets spiritual, social, emotional, physical, and intellectual needs of the resident · Asses resident characteristics (i.e., stages, sex, ethnic background, prior lifestyles, cognitive and functional abilities) and, in conjunction with other departments, plans and organizes program content · Monitor daily functioning of the neighborhood to ensure continuity of, and appropriate changes in the program · Assist in developing, implementing, and conducting in service training and education of care to all staff regarding memory care programs/activities working alongside the Director of Nursing and Life Enrichment Director. · Establish and maintain a sense of teamwork through effective communications, interaction, and team meetings. Develop and maintain cooperative relationships; inform and consult with staff regarding program and integrate with other services · Participate in support groups at the direction of the Life Enrichment Director · Assess the educational needs of staff regarding program and dementia-specific knowledge and works with the Life Enrichment Director to ensure appropriate education is provided · Market the program through involvement in community organizations and participates in the local Alzheimer’s and like associations · Maintain accurate and timely documentation that complies with state regulations and community policy · Work with management to develop and maintain written program objectives and procedures for implementation; method of evaluation · Serve as a role model for staff regarding care of dementia resident · In coordination with the nursing department and Director of Life Enrichment, perform a pre-admission assessment for each potential resident · Assist with the resident’s admission to ensure a smooth transition · Assist with the adjustment of the resident and family to the community; contacts weekly for the first month post-admission · Keep abreast of current research, new programs, and community resources which may benefit residents and families and makes referrals as appropriate to facilitate the resident’s use of resources, and to promote the resident’s increase level of social functioning · Assist residents in the maintenance and adequate supply of personal clothing and other personal items · Refer the resident/resident’s sponsor internal and external services that are available to the Director of Life Enrichment · Agree not to disclose assigned user ID code and password for accessing resident/facility information and promptly report suspected or known violations of such disclosure to the Executive Director Preferred Skills and Qualifications: · Must be an LPN · Experience with Alzheimer’s and other dementia individuals · Two years of previous experience in programming: including but not limited to: POC (plan of care programs, scheduling staff, coordinating meeting with POAs and families, planning activities and working with dementia residents in an assisted living environment · Background in nursing/ proving one on one care for seniors · Flexible schedule, including availability to work evenings, weekends and holidays as needed General Working Conditions: This position entails standing for long periods of time. The Assistant Nurse Manager (ANM) assists the Nurse Manager in leading a team of care providers to provide patient focused care through the integrated skills of all care team members.
