Job Summary: Essential Duties and Responsibilities Supports and promotes the mission and values of Covenant Health Ministry. Prepares patient for examination/visit. Obtains and thoroughly documents patient vital signs and reason for visit. Prepares patient chart for provider. Reviews current medications, allergies, treatments, height, weight, problem list and additional criteria as requested by provider. Reviews and documents patient history (immunization, screenings, etc.). Instructs patient in collection of samples and preparation for tests. Assists providers with patient examinations and procedures. Assists with the preparation of equipment/instruments Assists with patient education. Screens and documents patient calls determining the urgency of the problem; consults with provider regarding treatment and advises patient accordingly. Performs patient triage. Documents charts including returned phone calls, missed appointments or other clinical issues, with initials, date and time. Takes messages and assists providers with return phone calls. Calls in prescriptions per provider orders and protocols. Retrieves records and files/imports patient test results. Notifies patient of test results per provider direction thoroughly documenting patient outreach in a timely manner. Perform all required QCs (test and equipment) Preloads pertinent patient history in EMR. Performs tests as instructed (i.e., EKGs, lab, vision, hearing, PFT, non-stress). Administers allergy injections, medications, immunizations and other measured injectables per provider orders. Performs dressing applications and changes. Removes sutures. Performs venipuncture per provider orders. Prepares, cleans and stocks exam rooms. Schedules patient appointments and assists in scheduling patient tests. As a CMT, your main role will be to deliver care to our residents within a warm, comfortable, and home-like environment. Responsibilities Communicates questions, concerns, and/or discrepancies related to medication administration to the Resident Care Director (RCD). Communicates any significant change of mental or physical status of resident to RCD. Documents supervision of self-administration in MAR. Documents/ notifies RCD of administration of prn meds. Responsible for documentation on prn meds and affect. Recognizes and can assist in the prevention of elder abuse, neglect, and exploitation. Requirements Must have state Medication Technician License and demonstrate competency in accordance with the state-approved protocol in drug administration Successfully complete medication technician competency review (as supervised by RN / RCD) quarterly CPR certification required or ability to obtain within six months of hire The Medication Technician must complete a state approved training course in drug administration and receive a certificate of satisfactory successful completion. Assist residents with basic grooming needs. Serve meals, cue those residents who require it and assist residents in the dining room. Assist LNA's to provide fluids and nourishments to the residents. Maintain a safe, attractive and comfortable environment for the residents, with special consideration to bedmaking and closet cleaning. Answer resident calls for assistance promptly and refer their needs to an LNA or Nurse. Report to the Nurse immediately if the resident complains, has a change in condition, or whenever other special needs arise. Provide special services for the resident not needing professional skills. Carry out housekeeping functions on the unit that are not specifically assigned to the Housekeeping Department. Maintain helpful interpersonal relationships. Transport residents as required. Responsible for attendance at annual in-service program with regards to body mechanics, right to know and fire and safety procedures. Qualifications: The PCA will have a high school diploma (or equivalent) and have attained the legal age of employment in the state. Must be able to stand, walk, lift up to 50lbs with repetition. She/he is preferably attending a health occupations class or is interested in pursuing a health related career.
