Job Description Summary: An Infusion Nurse II is a licensed registered nurse who coordinates and provides intermediate direct patient care to patients in the home, or in an alternate infusion suite, to ensure patient safety with continuity and compliance under a physician’s plan of care. Prepares clinical documentation in real-time during visits and submits to the nursing department to comply with established timelines for billing optimization. Completes all documentation legibly and applies approved abbreviations and documentation error correction practices per Option Care’s policy. Coordinates discharge planning and prepares discharge summaries with patient instructions and thoroughly reports patient care needs, progress and goals when transferring care. Demonstrates compliance with agency operations, Option Care’s policies and procedures, professional standards, local, state, federal regulations/guidelines, and accreditation standards. Maximizes work efficiency through the use of computers and other automation technologies to validate plan of treatment orders, communicates patient care provided, and follows assigned visit schedule. Responds promptly and appropriately to patient requests. Initiates emergency procedures as necessary. Accepts accountability for own practice through ethical and professional conduct. We are seeking a dedicated Afterhours RN Care Manager to provide high-quality, compassionate hospice care outside standard business hours. This role ensures seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call hours. This is a full-time opportunity working every weekend assisting with visits, admissions and administrative support. About the Role The Afterhours Care Manager RN is responsible for assessing and coordinating patient and family needs during after-hours shifts and maintaining primary clinical oversight for assigned patients. This includes routine visits, admissions, death visits, urgent after-hours calls, and any visit assigned per the patient’s plan of care. This role serves as an active member of the Interdisciplinary Group (IDG) to ensure coordination between physicians, families, and the hospice team. Essential Functions Ensure appropriate delivery of hospice services to eligible patients and families. Conduct initial and ongoing assessments of physical, emotional, psychosocial, and spiritual needs. Support the IDG process through chart reviews, plan of care updates, and interdisciplinary communication. Develop and revise the Hospice Plan of Care in collaboration with physicians and IDG members. Educate patients and families on disease progression, symptom management, medications, and hospice philosophy. Communicate significant changes in patient condition promptly to hospice and attending physicians. Maintain accurate, timely clinical documentation including assessments, care plans, and notes. Assist with rescheduled, missed, or reassigned visit requests based on triage direction. Participate in calendar and scheduling modifications, including service codes and clinician assignments. Support administrative and compliance activities including workflow audits, survey readiness tasks, and documentation collection. Assist with TPE reviews, QAPI efforts, and regulatory compliance. Ensure infection control standards and universal precautions are consistently followed. Provide leadership and guidance to field staff; conduct supervisory visits when appropriate. Participate in required meetings, after-hours updates, and trainings. Maintain proficiency in pain and symptom management for hospice and end-of-life care. Responsibilities Assesses all patients assigned and establishes and implements the Plan of Care Adheres to policies and procedures addressing the scope of services outlined in both Infusion nursing services, pharmacy services and state laws and regulations Adheres to INS and IgNS standards of practice, CDC guidelines, scope and standards of practice, and code of ethics as defined by American Nurses Association Collaborates with pharmacy staff, sales partners, ordering provider and patient to develop and monitor goals of patients’ therapies Completes patient care documentation in place of service including patients’ home or AIS Manages vascular access devices, to include insertion of peripheral infusion devices and ability to draw labs via peripheral venipuncture Educates patients and caregivers Evaluates progress toward goals and makes recommendations to physicians for changes in the Plan of Care based on patients’ needs Coordinates nursing care with home health and other community agencies as indicated Participate in on-call rotation as scheduled per branch protocol Complete expense reports monthly tracking mileage and other miscellaneous incidental expenses Complete annual competencies and mandatory training within assigned due dates Supervisory Responsibility: No Qualifications Graduate of an accredited school of nursing; Bachelor of Science in Nursing preferred Valid nursing license in state(s) of practice Minimum of one (1) year of experience as a Registered Nurse Previous infusion and home health care experience preferred Extensive experience in IV therapy preferred Current CPR certification Flexible schedule with the availability to work a variety of shift models Ability to work effectively within a multidisciplinary team Effective verbal and written communication skills Valid driver’s license and auto insurance Must meet company standards for driving record as well as driver age requirement Percentage of Travel: 100%-Regular driving to patient’s homes for visits is required as part of this position **To perform this role will require constantly sitting, standing, walking, bending, reaching, crouching, stooping and typing on a keyboard with fingers. Competently handles multiple assignments often simultaneously, prioritizing work into manageable and measurable units.
