Description The Home Health Nurse Manager is responsible for the management and oversight of clinical staff for the licensed home care agency and nursing staff supporting home infusion services under the guidance of the Director of Patient Services, S/he ensures that personnel issues are managed in compliance with Department of Health (DOH) regulations and contractual agreements. S/he works in collaboration with clinical leadership, administrative leadership, providers of care and referrers to ensure that the service maintains the highest standards of patient care and is responsive to the needs of the patient and family and the organization. Responsibilities Patient Experience 1. Supervises all clinical staff of Mount Sinai Home Health LLC home care agency and nursing staff supporting home infusion services. 2. Oversees the agency orientation for all newly hired clinical staff including introduction to agency policy and procedures, clinical skills training and assessment, documentation and EMR requirements and competency evaluations. 3. Performs field supervision visits with all clinical staff on an annual basis at a minimum and remediation supervision visits as needed. 5. In this role, the successful candidate Responsible for using own clinical expertise to provide breastfeeding education and counseling. The Lactation Coordinator works collaboratively with other health team professionals to promote breastfeeding management. The Lactation Coordinator provides leadership to the patient care team by mentoring and sharing current evidence-based information and clinical skills. LOCATIONS AVAILABLE : BRONX and BROOKLYN POSITION OVERVIEW: The RN is responsible for the complete nursing care and performance of all routine medical procedures within the scope of the nursing practice. The RN is responsible for supervising LPN’s, AMAP and Direct Support Professionals (milieu counselors) in the performance of nursing tasks and activities. The RN must possess a working knowledge of NYS rules and regulations and JBFCS policies and procedures. The RN must be capable of supervising, understanding and working effectively with diverse populations. KEY ESSENTIAL FUNCTIONS: Provide direct nursing care as prescribed by the physician/Nurse Practitioner, in compliance with recognized nursing standards and State and Federal regulations. Uses nursing process to make continuing assessments of our client’s total health needs. Perform nursing assessments, by direct physical examination. Develop and implement an individualized plan for nursing service. The nursing care plan must be updated at least annually or whenever there is significant change in the client’s condition.
