Overview Case Manager is responsible for chart review to evaluate medical necessity for admission, continued stay, and the appropriate level of care. Monitors the care of assigned patients to assure quality cost effective utilization of resources and optimal outcomes. JOB SUMMARY: The MDS/Care Plan Coordinator is an experienced health care provider who ensures an accurate assessment and up-to-date care plan for all residents and that all records pertaining to resident care are consistent with the plan of care and are descriptive of the care rendered.
