Job Opportunity: Director of Nursing Services (RN) We are seeking a dynamic and experienced RN Director of Nursing Services to join our team. In this crucial role, you will be responsible for planning, developing, organizing, implementing, evaluating, and directing the Nursing Services Department. About Total Life Total Life connects adults 65+ with licensed clinicians for behavioral health, geriatric care and longevity services, covered by Medicare, Railroad Medicare and major commercial insurers. Since 2021 we've grown to more than 200 credentialed providers serving patients in 49 states. Most longevity medicine is built for healthy 40-year-olds. Ours is built for older adults. The Longevity Program combines a comprehensive geriatric assessment with chronic disease management, cognitive health, medication review and weight management, organized around five pillars of healthy aging: sleep, nutrition, movement, mental health and social connection. It works alongside a patient's existing care and never replaces their primary care provider. The role This is an in-person contract role. We're looking for nurse practitioners in the Connecticut shoreline (Groton-New Haven) area to lead longevity days at our partner senior living communities. Each day opens with a short educational talk for residents on healthy aging, using a presentation and speaker notes we provide. You then see the residents who sign up for one-on-one longevity visits. You'll be trained and supported by our Chief Medical Officer, a board-certified geriatrician who designed the program's clinical content. Your first site day is run alongside a member of our team. Schedule Site days are on weekdays. The presentation is the fixed part of the day. The rest depends on how many residents sign up. Expect roughly 4 to 8 hours per site day, a few times a month to start, with hours growing as bookings grow. Position Purpose: Our Home Care RN’s provide primary nursing care to home based care patients as ordered by the physician, and in accordance to the organization’s policies and procedures. The RN is responsible for the initial assessment and care plan development in collaboration with the Population Health Case Manager. They also provide functional support/supervision to Home Healthcare Aides and LPNs as needed and therapeutic intervention, overall care management and achievement of top decile client outcomes and patient satisfaction. Position Details: Schedule: Monday - Friday, 8 am - 5 pm, one weekend every 4 weeks and two rotating holidays per year. NO ON CALL REQUIREMENTS! Coverage Region: Enfield, Windsor Regions Bonus: $10,000 sign-on bonus paid out over two years. NOTE: 100% home care providing services in patients homes. What you will do: Plan and prioritize visits based on client needs and physician orders. Assess clients at admission and key intervals and document accurately. Develop and update care plans with measurable goals in collaboration with physician and client. Provide therapy using approved techniques, exercises, and modalities. Coordinate care and referrals with other disciplines as needed. Educate clients and caregivers on therapy programs and home exercises. Monitor progress and report changes promptly to the physician. Ensure compliance with laws, regulations, and agency policies. Complete documentation at point of care in a timely manner. Follow operating instructions to perform and document daily controls and calibration of equipment; maintain Quality Assurance/Quality Control logs for equipment troubleshoot breakdowns, perform preventive maintenance, and submit repair tickets as needed.
