Full job description There For You Home Care Services is looking to hire compassionate, self-directed Registered Nurses to provide skilled nursing care to patients in their homes. The Home Health RN assesses patient needs, develops and manages individualized care plans, delivers hands-on skilled care, and coordinates with physicians and the interdisciplinary team to help patients recover, manage chronic conditions, and remain safely at home. This role combines clinical expertise with strong documentation and independent judgment in a home-based setting. Responsibilities (include but are not limited to): Conduct comprehensive in-home patient assessments, including OASIS assessments at start of care, recertification, resumption of care, transfer, and discharge. Develop, implement, and revise individualized plans of care in collaboration with the physician and interdisciplinary team, ensuring alignment with physician orders. Provide skilled nursing services such as wound care, medication administration and management, IV therapy, catheter and ostomy care, disease management, and patient monitoring. Educate patients and caregivers on medications, disease processes, treatment plans, safety, and self-care to support independence and prevent hospital readmission. Coordinate care with physicians, therapists (PT/OT/ST), social workers, home health aides, and community resources. A Surgical Technologist at this level is new to the role of a Surg Tech and requires training in order to be fully proficient in all required surgeries on infants and pediatric patients. The Surgical Technologist I provides assistance during surgical procedures by passing sterile instruments to the surgeon. As directed by the surgeon, may hold retractors, sponge or suction operative site, and apply electrocautery within the surgical technologist’s scope of practice. Performs routine and delegated duties before, during and after surgery according to Connecticut Children’s standards, policies and procedures. This includes scrubbing and other related duties under the direct supervision of a Registered Nurse to maintain sterility during surgical procedures. Preoperative-20%: Assists Registered Nurse in gathering appropriate supplies for cases. Assists team members to place and position patients on the Operating Room table. Location Detail: 81 Meriden Ave Bradley Memoria (10003) As part of a person-centered care model, this role will assist in management of daily operations of an interdisciplinary care team. This role will ensure the delivery of outcome based , cost-effective care, by utilizing metric-driven decisions. The primary purpose of this role is to oversee all patient care services and personnel for their assigned team. This role will collaborate with all team members to achieve exceptional outcomes. JOB RESPONSIBILITIES • Assures the development of personnel qualifications and policies. • Identifies and facilitates professional development needs and competency of staff through home visits with staff to supervise and evaluate clinical performance. • Maintains compliance with regulatory agency policies and procedures. • Accountable for team performance in achieving desired clinical and operational performance measures. • Utilizes clinical software to identify opportunities for education, service utilization, patient progression, and team outcomes. • Assures that clinical and functional outcomes are achieved at the highest level possible. • Resolves patient care problems collaboratively with internal and external customers. • Oversees daily patient assignments for clinical staff. • Coordinates referrals and ensures timely start of care and service delivery. • Ensures patient needs are continually assessed. • Ensures individualized plans of care are developed, implemented, and updated. • Ensures coordination of patient care and communication with physicians. • Determines staffing needs and coordinates hiring. • Hires, trains, coaches, and evaluates staff. • Reviews and analyzes team and patient metrics. • Participates in preceptor programs. • Participates in performance improvement, huddles, IDT, and QAPI activities. • Performs other duties as assigned. Qualifications Minimum Requirements: • Associate Degree Registered Nurse • Home Health or Hospice field experience • Current Connecticut RN license Preferred Requirements: • Bachelor's Degree in Nursing • OASIS Certification • Case Management Certification
