Overview Oversees, supervises, coordinates, facilitates, and supports the Hospice After Hours care delivery activities, including field staff supervision. Provides support and direction to families in accordance with the patient’s Interdisciplinary Plan of Care and Hospice Care mission, objectives, policies, and procedures. • Takes referral information and triages for immediate response, dispatches the on-call nurse to do the evaluation/ admission. Contacts Physician for verification of prognosis and appropriateness for Hospice/Palliative care. Contacts and interface with team members, primary physician, team physician, administrator, and other staff to ensure patient’s and family’s needs are met. • Works directly with outside vendors and care delivery partners, to ensure when patients are transferred to General Inpatient Facilities, Emergency Departments, and Paramedicine Activation, that documents and patient information to ensure continuity of care and most effective care based on patient/caregiver goals and clinical needs. • Provides clinical directions to patients/caregivers in accordance with Hospice guidelines. Obtains and receives verbal orders from team/physician and documents care using standard form. Evaluates and documents effectiveness of directions through telephone follow-up with patients/caregivers. Provides team/physician with documentation and implements any changes, as directed. • Obtains and reviews daily report from each team on significant patient care situations that may require contact/follow-up during course of shift. Documents all calls received using high degree of accuracy and detail to ensure team/physician is provided with all relevant and necessary information. Completes case conference form on all calls received and faxes to respective team. Supervises on-call Nurses: Monitors/evaluates performance, provides guidance, recommends hiring and termination, as needed. Determines/Deploys on-call Nurse for home visits, provides necessary information and follows-up regarding status of visit and to obtain necessary report. • Provides verbal report on significant events, plan of care changes and other patient/caregiver issues requiring further action and/or follow-up to each respective team. • Participates in Interdisciplinary Hospice Team meetings, management meets and serves on relevant committees for the purposes of information exchange, team collaboration, development of procedures and documentation tools, development of quality and training. • Understands/implements/complies with Hospice Care Mission, objectives, policies, and procedures to ensure Hospice Care/patient/caregiver needs are met. • Participates in special projects and performs other duties as assigned. Qualifications Licenses and Certifications: Hospice and Palliative Nurse (CHPN), Hospice and Palliative Credentialing Center preferred License and current registration to practice as a Registered Professional Nurse in the State of New York required Education: Associate's Degree in Nursing required Bachelor's Degree in Nursing preferred Work Experience: Minimum of two years of clinical nursing experience in Acute Care (Med Surg, ICU), Clinical Crisis Management, Hospice or Palliative Care required Clinical expertise and critical thinking skills required Effective oral, written, and interpersonal communication skills required Demonstrated expertise in clinical care management required Knowledge of personal computer operations including MS Office applications required Experience in health care management preferred Pay Range USD $93,400.00 - USD $116,800.00 /Yr. Job Details St. Francis Hospital Critical Care Advanced Practice Provider (APP) is a valuable team member. Primary responsibility is the management of critically ill and complex (multi-organ) patients in an intensive care setting. This setting includes the management of patients in the Cardiothoracic ICU (CTICU), Surgical ICU (SICU), Medical ICU (MICU) and Intermediate Care Unit (IMCU). The APP is under the direct supervision of an in-hospital 24/7 Critical Care board certified Intensivist program. The APP performs physical exams, collects and documents data, conducts approved diagnostic and therapeutic procedures, orders and schedules laboratory studies and professional consultations, prescribes appropriate interventions and medications, and provides direct patient care services in a specific ICU. Responsibilities: Provide direct clinical services for critically ill adult patients with multi-system disease /failure, according to established protocols and evidence-based clinical practice standards. Obtain patient health histories and complete physical examinations. Document findings, noting pertinent normal and abnormal findings. Order and interpret appropriate diagnostic and laboratory tests. Explain the necessity, preparation, nature and anticipated effects of procedure(s) to patients, patient's family, staff, and healthcare interdisciplinary team members. Perform approved therapeutic or diagnostic procedures based upon patient's clinical status. Formulate and implement a treatment plan for acute illness management in collaboration with the designated primary supervising physician and/or the intensivist. Utilize prescriptive privileges as allowed by NY licensure. Assess patient for change in status and institute appropriate interventions. Initiate necessary emergency interventions to stabilize patient. Participate in our Clinical Education and Science Simulation center for onboarding and strengthening of both didactic and procedural skill sets. Develop procedural skill sets and continued competency including placement of central lines, pulmonary artery catheters, hemodialysis catheters, trans-venous pacing wires, arterial lines, PICC lines and mid lines. Other complex management includes mechanical support devices such as Impella, Left Ventricular Assist Devices (LVAD), and Extracorporeal Membrane Oxygenation (ECMO). Description Job Title: Patient Care Associate-Rehab-Full Time Day 7am-3:15pm-MSM As an integral part of the clinical care team, the Patient Care Associate is responsible for patient care and support activities promoting patient focused care within the Specialty Service. The Patient Care Associate supports the registered nurse in patient care activities, such as: assisting patient with activities of daily living, collecting specimens and performing procedures; maintaining a safe, clean environment; and other duties as needed. Responsibilities A. Clinical/Technical/Service • Demonstrates the ability to perform clinical/technical/service/administrative tasks • Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. • Performs and records patient’s vital signs, such as blood pressure, pulse, temperature and respirations. Documents intake and output and other data as assigned • Obtains patient’s rating on pain scales as a fifth vital sign in all areas except Pediatrics • Performs clinical procedures, such as specimen collection including blood glucose, venipuncture, EKG, application of compression devices, post-mortem care, and maintenance of skin integrity. • Sets up for selected procedures and assists RN’s accordingly. • Assists patient with activities of daily living, such as feeding, bathing and/or toileting. Orients Patients to clinical environment. Assists patient with ambulation and all levels of mobilization. • Assists with dietary functions, including delivering and collecting patient trays at bedside, passing menus to patients and assisting them with menu completion, mixing nutritional supplements, reporting variation in eating patterns to RN/Dietitian, and maintaining accurate and timely systems for transmission of patient dietary orders and changes • Maintains a safe, clean and functional environment, including ensuring for the comfortable Movement of the patient. Assists the patient in room maintenance, such as bed-stripping and trash removed, when necessary • Performs on-to-one observation or close observation status of patients, e.g. safety, elopement risk, or suicide prevention. • Maintains “isolation status” for patients under direction of an RN and according to set procedures • Performs scheduled hourly care checks/ rounding on patients with emphasis on caring for toileting needs, comfort, and positioning and pain awareness. Description Position Title: Licensed Practical Nurse-Institute for Advanced Medicine-Mount Sinai West-Full Time-Days Position Summary: Participates in the application of the nursing process to an assigned group of patients. Clinical/Technical/Service: Participates in the application of the nursing process to an assigned group of patients. Demonstrates a competent level of approved skilled nursing care tasks and procedures using approved technical skills related to vital signs, pulse oximetry, venipuncture, starting standard peripheral IVs only (not midlines or long peripheral lines), urinary catheter insertion/catheter care; and simple dressings, remove/discontinue a peripheral line. Administer IV fluids, provided completion of annual competency IV therapy training. Monitors patients receiving blood and blood products and TPN under the direction of the RN, who administers the transfusion. Makes recommendations in the application of the nursing process. Participates in planning, and administers direct patient care, including activities of daily living (ADL). Performs point of care testing including blood glucose.
