Job Responsibilities Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse. Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient. Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails. Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies. Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry. Feeds and assists with meals. Obtains temperature, pulse and respiration as supervised by the Hospice Nurse. Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information , including the date and time of the report and the person receiving the information. Leads staff participation in quality and safety initiatives and mandated compliance measures. Demonstrates accountability for all unit quality and safety practices and compliance, and ensures required evaluation of staff competencies is maintained. Operations and administration Supports implementation of the mission, vision, goals and objectives of the department, throughout the clinical service and individual patient care units. Provides administrative and clinical oversight to ensure that all activity (staffing, support services, inter disciplinary collaboration, resources and financial viability) supports the goals of the organization. Works collaboratively with physician dyads and all members of the healthcare team to promote patient care and regulatory standards. Prioritizes recruitment and retention strategies in accordance with system and local hospital retention goals. Maintains a nursing unit in compliance with all regulatory agency mandates, including the regulations of NYS DOH and The Joint Commission. Oversees 24 hour patient care delivery, throughput, staffing and processes to ensure efficient and effective operations in all clinical specialties. Implements and monitors unit salary and supply budgets, including capital equipment allocation, budget variances, management of OT and vacancies according to MSHS financial goals. Monitors availability and maintenance of nursing equipment and supplies Maintains position control of established positions and manages the recruitment and retention of qualified staff. Maintains patient/employee confidentiality in the management of information. Evaluates nursing staff performance and mentors, coaches, counsels and disciplines staff as needed. Evaluates all newly hired/transferred staff and participates in the decision about passing the probationary period. Participates in the ongoing and completes the annual performance appraisals for all staff. Completes performance improvement plans of employees as needed and shares feedback with the Associate Directors/Directors and Senior Directors accordingly. Interprets and implements the associated collective bargaining agreements and System and Hospital policies and procedures. Performs disciplinary actions as necessary with collaboration with Nursing and Labor Relations. Acts as a management witness in disciplinary activities. Insures all staff have met their orientation goals/weekly progress reports and annual competency requirements and participates in the Competency Planning Tool for unit/service. Conducts regular staff meetings to ensure communication with staff about progress, changes and compliance with all hospital policies and procedures. Maintains records of minutes for staff team meetings. Encourages, recognizes and rewards staff participation in quality and safety initiatives, compliance and successes. Facilitates organizational change initiatives by using effective strategies. Leads a shared decision making model by establishing a structure of shared governance, just culture and staff engagement in practice councils, departmental meetings and initiatives. Responds to protest of assignment in a timely manner as detailed in the collective bargaining agreement and discusses the resolution in unit staff meetings.
