Description The Nursing Attendant is an individual who functions in a support role to the Registered Professional Nurse by providing direct patient care and related activities as assigned. Responsibilities Assists the nursing staff in performing selected nursing procedures, treatments, and functional tasks to specific patients including vital signs: Temperature, Pulse, Respirations, Blood Pressure and Pain Rating. Performs procedures associated with activities of daily living, ambulation, mobility, and personal hygiene of the patient. Observes, reports, and records routine and specific patient data, behaviors and responses to treatment on the appropriate flow sheet/EMR. Reports pertinent information and observations regarding patients to the registered nurse Performs procedures related to assuring a safe environment for patients, staff and others. Performs procedures associated with the admission, transfer and discharge of patients. Performs procedures associated with the maintenance of food and fluid intake. Performs procedures associated with excretion of body waste and or loss of fluid. Provides for safe and comfortable lifting, positioning and ambulation of patients in accordance with the principles of good body mechanics. Utilizes mechanical devises as necessary Performs simple applications of heat and cold. Provides care to the body of deceased patients in preparation for removal to the morgue. Provides support to the family/significant others. Secures property. Obtains, assembles and maintains equipment for use during diagnostic and therapeutic procedures. Assists members of the health care team with physical exam and other procedures. Conducts purposeful hourly rounding for assigned patients noting: Pain, Position, Proactive Toileting and Possessions Participates in the patient belonging process, including reporting of valuables to the RN and documenting accordingly. Transports patients by wheelchair, stretcher or bed, and escorts ambulatory patients. Performs errands away from the area of assignment to implement the care of patients Including transporting stat specimens and/or obtaining supplies as necessary. Maintains a clean uncluttered and tidy environment. Maintains equipment and cleans as necessary. Stocks supplies. Works in a safe manner and provides for the safety of patients and other employees. Utilizes aggression control measures in the care of patients who are verbally or physically threatening. Assists the RN with the application of restraints as required. Performs patient observations for Safety, constant/One to One and/or every 20 minutes checks of assigned observation status. CHHA Field RN (UFT union) for Manhattan area

Job Description The CHHA Department is currently seeking a full time CHHA Registered Nurse to join their team! Eligible newly hired UFT nurses will receive a $6,000 hiring bonus, distributed in two phases: the first installment of $3,000 will be provided after successfully completing six months of employment, followed by a second $3,000 payment upon reaching one year of service. Job Summary: Responsible for providing and coordinating multidisciplinary health services for patients in the community. Works with the physician and other team members in implementation of plan of care. Duties And Responsibilities Provides and coordinates cost-efficient patient care that is and reimbursable under the patients’ insurance plan and approved by a physician. Verifies insurance plan information, crosschecks patient Insurance ID card at home. Accepts case assignment depending on clients’ needs regardless of geographic location. Case assignment is all inclusive of CHHA (Certified Home Health Agency) requirements, including initial assessments, revisits and documentation on each day of service. Travels to patients’ homes and/or other facilities using approved transportation to deliver direct care to patients Admits patients in the program as needed and in a timely manner. Ensures in-person completion of the Oasis that accurately reflects clinical and functional status of the patient. Uses diagnosis coding skills to accurately rate the acuity of each Evaluates patient diagnosis and works with Director, Patient Services and physician to assign appropriate primary diagnosis. Participates in bi-weekly and PRN meetings to report adherence to frequency and duration of services as per physicians’ order. Reports patients’ progress to team in order for discharge date to be anticipated and set. Communicates frequently with PT/OT to ensure adherence to therapy at the frequency and duration ordered by the physician. Acts as a coordinator of the multidisciplinary team providing care to patients. Establishes a plan of care with input from appropriate multidisciplinary health team members based on goals mutually acceptable to the patient and family. Documents content and process of all services provided. Monitors the effect of the care plans on the patient health status and informs physician and other personnel of changes in the clients’ needs. Appraises care provided, revises goals and interventions to patients’ care as needed. Educates the patient and family regarding the disease process, self-care techniques and prevention strategies. Initiates and maintains written and verbal communications to assure optimal patient care. Provides supervision of workers providing health and personal care services through visits to patients’ home. Maintains updated Home Health Aide (HHA) plan of care and reviews same with HHA and patient/family. Communicates with HHA on a regular basis regarding interdisciplinary plan of care, patients’ goals and progress. Documents initial and bi-weekly supervision of HHA services. Plans, facilitates and documents patients’ discharge from the program. Manages caseload as assigned in Manhattan. Skills & Requirements Specialized Skills: Demonstrates competency in the provision of nursing care. Demonstrates knowledge of the PPS Medicare system. Demonstrates knowledge of the Oasis and impact on payment. Demonstrates flexibility and the ability to prioritize responsibilities. Demonstrate ability to document and coordinate patient care. Ability to assess, plan and intervene in patient care activities appropriate to patients’ age, capabilities, disabilities, temporary impairments, emotions, stressors, culture, and individual station. Requirements License and current registration to practice as a Registered Professional Nurse in New York State. Graduate of an accredited school of nursing, BSN preferred. A minimum of one year experience in hospital and or minimum of one year experience in a CHHA preferred.

In this role, the successful candidate Assists in implementation of the nursing process by performing direct and indirect patient care to assigned patients under the supervision of the Registered Professional Nurse. Assists in maintaining an orderly and safe patient care environment and unit. Job Responsibilities: Assists with Admission/Transfer/Discharge of patients: Orients patient to hospital environment. Provides patient with toileting supplies. Assists patient to remove clothes, complete property check list and safeguard personal property through Security. Assists in transfer of patients by gathering belongings, completion of clothing list and assisting in moving the patient. Prepares bedside unit to receive patients. Discharges: Assists patient to dress and pack personal belongings. Escorts patient as necessary and remains with the patient until he/she has left the building. Conducts patient rounds for toileting, positioning, comfort, safety and personal needs. Answers patient call lights and responds to patient needs promptly. Assemble O2 (e.g., nasal cannula) and suction set- ups. Provides basic hygiene to patients: bathing, AM/PM care, mouth care, denture care, hair care, nail and foot care, shaving, assists patients with bedpan, urinal and/or commode. Prepares patients for meals including: Clearing overbed tables and placing them within reach of the patient. Positioning patient correctly for eating/tube feedings. Performs post mortem care. Performs safety observations for patients on continuous observation or in restraints, per policy and as directed. Reports any changes observed in the patients condition to the RN. Responds to and meets patient/family needs and customer concerns as soon as possible with compassion and understanding. Fee For Service Registered Nurse for Manhattan area

Job Description The New Jewish Home Certified Home Health Agency (CHHA) provides home based care and rehabilitation therapy for patients recovering from an acute episode or other adverse health event. The New Jewish Home has long experience delivering rehabilitation and home care services. Our goal is to help each patient get well, stay well and return to health and independence as quickly as possible. We are currently looking for a CHHA Fee For Service Nurse who can cover the Manhattan Area. Weekend hours also available. Training during the week. Job Summary: The CHHA Fee For Service Registered Nurse will provide and coordinate multidisciplinary health services for patients in the community Conduct assessment and collaborate with the Physician and other team members on the implementation of care plan with a goal for client to be discharge from program. Further duties include ensuring that all documentation required are completed within regulatory guidelines including Physician Orders (485) and Oasis which must accurately reflect clinical and functional status of patient in order to follow reimbursement policies. Skills & Requirements Job Qualifications: Valid NYS RN License Graduate from an accredited school of nursing, BSN preferred. A minimum of one year nursing experience in a LTHHCP and a minimum of one year experience in a CHHA preferred.
