Description Position Summary: Provides direct and individual nursing care to assigned residents based on nursing standards and under the supervision of the Director of Resident Care (DRC). Also maintains compliance with all state and federal regulations and the standards of HRA. Essential Functions: Ensures quality and safe delivery of health services to residents Implements plan of care formulated by physician and DRC Provides accurate and timely documentation of care given Assesses and provides resident and family/care giver information pertinent to diagnosis or plan of care Administers medication per physician orders. Documents properly on MAR. Observes for any adverse reaction and notifies physician accordingly Assembles resident charts, resident records and checks that all necessary forms are complete and accurate Prepares incident/accident reports for residents as needed per policy. Notifies physician and family post incident/accident per policy and documents appropriately Maintains safe and healthy working environment and uses safe work methods and procedures in accordance with clinical standards Assists resident in learning appropriate self-care activities Organizes and monitors associates and material resources necessary for meeting the needs of the residents under his/her care Recognizes emergencies and life-threatening situations and initiates appropriate action(s) Maintains confidentiality and resident rights regarding all resident personal information Performs any other duties requested by supervisor The ability to work in a safe and alert manner The ability to take ownership for associate’s safety and the safety of the residents Must disclose any medication that might impair associate’s ability to perform the job safely or competently. Functions to advance the clinical mission and goals of the Georgetown Transplant Institute through collaborative relationships within the Medical Center. About this Job: General Summary of Position Provides professional nursing care for transplant patients following established standards and practices. Functions to advance the clinical mission and goals of the Department through collaborative relationships within the Medical Center. Primary Duties and Responsibilities Performs professional nursing service in inpatient/outpatient clinical settings including but not limited to: patient assessment teaching discharge planning and telephone triage. The Lead APP plays a key leadership role in coordinating daily operations for the trauma APP group, ensuring the highest standards of clinical care, teamwork, and professional development are achieved across the service. In this role, the Lead APP provides direct oversight of APP scheduling, workload distribution, and staffing, while fostering a culture of collaboration and excellence. They serve as a clinical expert and mentor, providing education, feedback, and coaching to support APP growth and performance. The Lead APP also acts as a liaison between APPs, trauma surgeons, nursing, and hospital leadership to ensure seamless patient care and communication throughout the continuum of trauma services. This position maintains an active clinical practice, performing comprehensive assessments, developing treatment plans, and managing acute and critically ill trauma patients.
