PURPOSE AND SCOPE: The professional registered nurse Outpatient RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. This may include delegation of appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs and Patient Care Technicians. As a member of the End Stage Kidney Disease (ESKD) health care team, this position participates in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations. PRINCIPAL DUTIES AND RESPONSIBILITIES: All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations. · Performs all essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor or in collaboration with other Registered Nurses. · Performs ongoing, systematic collection and analysis of patient data pre – during – post hemodialysis treatment for assigned patients and documents in the patient medical record, makes adjustments or modifications to treatment plan as indicated and notifies Team Leader, Charge Nurse, Supervisor or Physician as needed. · Assesses, collaborates, and documents patient/family’s basic learning needs to provide initial and ongoing education to patients and family. · Directs and provides, in collaboration with direct and ancillary patient care staff, all aspects of the daily provision of safe and effective delivery of hemodialysis therapy to assigned patients. · Administers medications as prescribed or in accordance with approved algorithm(s), and documents appropriate medical justification and effectiveness. · Initiates or assists with emergency response measures. · Serves as a resource for health care team, participates in staff training and orientation of new staff as assigned. · Ensures correct laboratory collection, processing and shipping procedures are performed and reschedules missed or insufficient laboratory collections. · Identifies expected outcomes, documents and updates the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team. · Ensures patient awareness related to transplant and treatment modality options. · Required to complete CAP requirements to advance. · Performs all other duties as assigned by Supervisor. EXPERIENCE AND REQUIRED SKILLS: · Entry level for RNs with less than 2 years of Nephrology Nursing experience as a Registered Nurse. · Chronic/acute hemodialysis experience (preferred). In this role, you will provide anesthesia care for a wide variety of surgical and procedural cases while working alongside anesthesiologists and fellow CRNAs in a team-based care model. All surgical specialty services and trauma are provided except burns and organ transplant procedures. Population includes pediatric, adolescent, adult, and geriatric patients. Number of beds and number of patient treatment areas: 18 beds. Average about 250 cases Monday - Friday. Level II trauma center. One of the busiest ERs on the West Coast! What you'll do Prepare and maintain a sterile surgical field by organizing instruments, equipment, and supplies in accordance with aseptic technique and hospital protocols. Conduct safety checks and ensure that surgical instruments and equipment are functioning properly before procedures begin. Actively support the surgical team by anticipating the needs of the surgeon and providing the correct instruments and supplies throughout the procedure. Participate in key patient safety protocols, including time-outs, medication labeling, hand hygiene, and post-procedure debriefs. Assist in patient positioning using safe body mechanics, and support dressing application, medication preparation on the sterile field, and proper specimen management. Take initiative during unassigned time to support unit needs such as stocking supplies, updating preference cards, and preparing rooms for upcoming cases. Foster a collaborative team environment by mentoring new staff, communicating clearly with team members and patients, and participating in process improvement efforts using Lean methodologies. Manage the storage and transport of surgical specimens and assist in ordering and stocking non-controlled medications and supplies for the unit. Required qualifications Basic Life Support (BLS). And will follow any one of three paths: Path I: Completion of an Accredited Educational Program Completion of a surgical technology educational program accredited by a national accreditation organization that is approved by the Oregon Health Authority; and Holds a current surgical technologist certification issued through the National Board of Surgical Technology and Surgical Assisting (NBSTSA); or obtains surgical technologist certification through the NBSTSA within twelve months of program completion. Path II: Completion of a US Military Surgical Technologist Training Program Provides documentation showing completion of a training program for surgical technologists in the Army, Navy, Air Force, Marine Corps, or Coast Guard of the United States or in the United States Public Health Service Commissioned Corps; and Demonstrates that sixteen hours of approved continuing education was completed every two years. Path III: Experience Provides documentation demonstrating practice of surgical technology during at least two of the years between January 1, 2014, and January 1, 2017; and If experienced in the state of Oregon, must also demonstrate that sixteen hours of continuing education was completed every two years since 1/1/2017. Caregiver Job Duties Our caregivers support their clients in activities of daily living such as (but not limited to): Household chores (cleaning, laundry, dishes, etc.) Cooking and/or serving meals Helping clients bathe, dress, and groom Providing companionship through daily activities and hobbies Driving clients (as needed) to the store or appointments Monitoring and reporting on their condition Additional Information Our hiring process is fast and simple!
