Location: Henry Wade Juvenile Center PRIMARY PURPOSE Responsible for providing total nursing care for assigned patients, under the guidance of a Registered Nurse, in order to ensure quality care. MINIMUM SPECIFICATIONS Education Must be a graduate of an accredited school of vocational nursing and/or licensed by the Texas Board of Nursing as a Licensed Vocational Nurse; or have a valid Compact LVN License. May assist with developing annual goals and objectives for the nursing area that support the mission and objectives. Conducts a comprehensive chart review with focus on inadvertent diagnostic findings requiring follow up testing and closing gaps in care. Assists patients and families with health system navigation, making appointments, patient advocacy, determination of community resources to reduce barriers to care, and support set-up and initiation of electronic communication modes (email, MyChart, etc.).
