Through the assimilation and application of evidence-based information, the NCM designs, implements, and evaluates client treatment plans by coordinating, delegating and participating in the care provided by the health care team. The NCM is responsible for working with the health care system to promote an environment that will empower his/her assigned patients to manage their illness(es) and achieve optimal outcomes. The NCM will provide and manage care of assigned community clients. Manages evidence-based practices and continuous process improvement initiatives to meet and exceed quality measures and enhance the exceptional care provided to patients. Manages the quality of care by collaborating with a multidisciplinary team to coordinate, evaluate, customize, and deliver a holistic patient treatment plan that promotes and supports recovery.
