The existing CTICU APP team is now expanding in response to unprecedented growth. The Cardiothoracic Surgery Intensive Care (CTICU) Advanced Practice Provider (APP) is a member of the Cardiothoracic Surgery CTICU multidisciplinary team that includes physicians, residents, fellows, and RNs providing intensive care services to patients and families. The CTICU APP role promotes independent clinical practice within a highly collaborative and established team model and is well supported with dedicated 24/7 in-house CTICU intensivist support. Responsibilities The successful candidate will be responsible for providing independent and collaborative patient management from admission to the CTICU through transfer of care to the acute care setting. The Platinum Inpatient ARNP/PA-C Is a Nurse Practitioner Or Certified Physician Assistant Who Provides Evaluation And Medical Management To Inpatient Medical Oncology And Hematology Patients At The University Of Washington Medical Center (UWMC). The ARNP/PA-C Provides care to patients admitted to the hospital for cancer-directed therapy, acute illness, and complications related to therapy, underlying disease, and concurrent illness. Performs clinical assessment and management of medical needs, performs procedures, participates in goals-of-care discussions, and consults with professional peers working collaboratively within an interdisciplinary team to coordinate care. All clinical responsibilities are performed within the scope of practice, institutional privileges and under the supervision of an attending physician. ALTERNATING WEEKENDS Job Summary: Provides comprehensive skilled nursing care and services to patients in the home setting. Works on an interdisciplinary team to provide care in the home health, palliative care and hospice programs. Accountable for the assessment, development of plan of care, education and consultation related to pain and symptom management as well as adjustment to changes in health status. Plans for the return of patient care to primary Health Care providers as indicated. Essential Responsibilities: Provides nursing interventions requiring substantial and specialized nursing skill, and initiates appropriate preventive and rehabilitative nursing procedures. Follows through the plan of care as developed to: provide appropriate instruction to patient/caregiver about health care, disease management and adjustment to illness; direct and supervise the care and services of LPN and HHA in the home setting; perform treatments; instruct patients/families in self care activities and home safety; and competently perform skills/tasks per plan of care. This includes pain and symptom management in the home. Performs skilled procedures to include injections, dressings, catheter insertion and care, colostomy and tracheotomy, central line IV care and blood draws. Identifies educational needs. Instructs patient according to the plan of care. Provides care and case management with interdisciplinary team. Communicates and collaborates with other interdisplinary team members and with the patient family in planning and implementing care. Records accurate and pertinent data in the electronic medical record in a timely manner, according to documentation guidelines. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items or baked goods that they themselves prepared 7. Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. 8. Reporting any complaint of pain or other symptoms or changes in the patient's condition to the RN Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues. 9. Assisting in ambulation and exercise as instructed by the hospice nurse or therapist. 10. Provide normal range of motion and transfers/positioning as directed. 11. Assistance with self-administered medications as allowed by state regulations, such as “reminders.” 12. Offering and assisting with bedpans and urinals. Providing assistance as assigned with light laundry needs, emptying waste baskets and other light homemaker tasks. 13. Performing range of motion and other simple procedures as ordered with appropriate completed/demonstrated skills competency. 14. Providing respite for patient's/family/caregiver when on-site, as appropriate. 15. Keeping patient's living area clean and orderly, as assigned. 16. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. 17. Participates in quality assessment performance improvement teams and activities. 18. Respectful of patient and family/caregiver environment and patient's personal needs. 19. As a mandatory reporter, reports failure to comply with the requirements of the Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 20. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. Makes independent judgments, decisions, and takes action regarding a wide range of routine and complex staffing and unit workflow issues. Collaborates with appropriate disciplines to coordinate continuity of patient care from admission through discharge in support of the medical and nursing plans of care. Primary Duties: 1. Performs specific leadership functions related to clinical practice, education, and/or management as delegated by and in collaboration with the Nursing Executive Director. Provides leadership for unit and departmental activities that promote the following: Delivery of quality nursing care A culture of service excellence and acts as role model for patients are first behaviors. Effective problem solving and communication. Fiscal responsibility and accountability for unit performance 2. Provides support to the unit management team to ensure strong workflow processes and infrastructures that promote efficient and effective quality patient care. Support is provided to all staff any day and any shift as determined by need/demand. Emphasizing patient safety, staff safety, organizational integration, and service excellence the ANM provides leadership and resources that promote the following: Assists in delivering effective staffing schedules that take into account staff experience, skill mix, preference, and core matrix. Efficient and effective timesheet/payroll management processes and practices Timely clinical problem solving and communication of clinical issues at all levels (peer, staff, management, and physician) Effective interdisciplinary and interdepartmental communication, collaboration and service coordination Effective integration of new technology and organizational initiatives Effective management of staff practice and professional behaviors using effective interpersonal skills and Just Culture™ methodology. 3. Performs other duties as assigned. License, Certification, Education or Experience: REQUIRED for the position: Bachelor of Science in Nursing (BSN) and/or Master of Science in Nursing (MSN) Licensed as a Registered Nurse in the State of Washington or holding a multistate Nurse Licensure Compact (NLC) license 4 years of experience in an obstetric inpatient setting (labor & delivery, postpartum, OB emergency dept and/or OB operating room) 2 years of experience in leadership role in an inpatient care setting such as Charge Nurse, Mentor, or Educator Current Healthcare Provider BLS certification by date of hire NRP required within 3 months of hire date Knowledge of Federal, State, and JC regulations and requirements DESIRED for the position: OB applicable nursing certification Previous experience as circulating or scrub nurse in an OB operating room setting Demonstrated understanding of financial management and a commitment to the provision of cost-effective health care.
