POSITION DESCRIPTION: Certified Medication Aide is under the direct supervision of the charge nurse. The charge nurse will follow up with the CMA to ensure the med pass is completed timely and accurately to assure that the highest degree of quality resident care can be maintained at all times. QUALIFICATIONS : Ability to communicate effectively in English through both speech and writing. Previous patient care experience preferred. Current certification for Medication Aide. RESPONSIBLITIES Reports promptly any change of patient condition or other concerns to Patient Care Supervisor. Follow charting guidelines for PRN documentation. Reports medication reorder. Participates in all assigned meetings, trainings, education and in-service as required. Inventory documentation of drugs on appropriate forms. Current certification for Medication Aide.

As a Field - Based Case Manager, you'll wear many hats and you will be visiting members in the hospital before discharge possibly with new medications or diagnoses. You'll assist members with safe, effective transitions from care environments to where they live. You may also act as an intermediary between providers and members - serving in numerous roles, such as educator, evaluator, service coordinator, community resource researcher and more. The result?
