Description Provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. Responsibilities 1. Fulfills patient care responsibilities as assigned which may include: checking schedules and organizing patient flow; accompanying patients to exam/procedure room, assisting patients as needed with walking, transferring, dressing, collecting and processing specimens and point of care testing, preparing for exam, etc. Assists physicians with various procedures, takes vital signs, relays instructions to patients and or families; answers calls and provides pertinent information. 2. Fulfills clerical responsibilities as assigned which may include: sending and receiving patient medical records; obtaining lab/x-ray reports, hospital notes,referral information, etc. Completing forms and requisitions as needed, managing charts to ensure information is completed and filed appropriately. These responsibilities may be carried out using the electronic medical record (EMR) system or manually as necessary. 3. Enters orders into the electronic medical record (EMR) system or prepares prescription refill requests on behalf of the physician for the review and approval by the ordering physician. 4. Inventories, orders or re-stocks medical supplies as applicable. 5. Provides set-up of examination room and instruments in accordance with proper sterilization techniques established by hospital and infection control policies. 6. Maintains logging system as required for refrigeration of medications, point of care testing, sterilization process, crash cart, point of care testing or others as needed. 7. Checks expiration dates on all medications and medical supplies disposing appropriately as per institution policy. 8. Maintains a safe, secure, and healthy work environment by following standards and procedures; complies with legal regulations. 9. Complies with established personal protective equipment requirements necessary for protection against exposure to blood and other potentially infections body fluids, chemical disinfectants, radiation and other hazardous substances. 10. Assists in the maintenance of medical charts (filing, Op Reports, test results, home care forms). 11. May schedule surgeries/procedures or work in conjunction with Surgical Coordinator verifying times with patients; preparing charts, pre-admissions and consent forms as necessary. 12. May be required to perform proficient phlebotomy duties. 13. May perform appointment scheduling and registration for patients as well as updating patient demographic and insurance information. 14. May maintain, adjust and confirm patient office schedule. 15. May check and verify and update insurance information utilizing internal or web sources. 16. May perform and verify pre-certification for procedures as necessary. 17. Mentors less experienced Medical Assistant staff. 18. Performs other duties as assigned. Qualifications High School Diploma/GED with completion of Training Certificate from an approved Medical Assistant certificate course. Certified Medical Assistant preferred. Description The Medical Assistant I provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. This individual is also responsible for assisting with clinical, administrative and organizational tasks associated with patient care coordination. Responsibilities 1. Assists the provider during patient visit via obtaining vital signs (height, weight, blood pressure, pain scale etc) and documenting vital sign results and transcribing medication information into the electronic medical record (EMR). 2. Obtains prior authorizations for prescriptions and durable medical equipment 3. Drafts prescription requests in EMR and sends to provider for review, completion and processing 4. Performs Phlebotomy services inclusive of collecting and processing blood, urine and stool specimens and use of a centrifuge for spinning bloods 5. Performs POCT (w/the exception of glucose) and documents results in EMR for provider to review 6. Performs daily Quality Control for all POCT equipment 7. Orders clinical supplies when necessary 8. Maintains and stocks exam and consult rooms with clinical supplies and clerical forms used by Providers 9. Assists providers with procedures by preparing supplies and equipment and acting as a chaperone as needed. 10. Perform EKGs when ordered by provider 11. Performs pre-visit planning for Diabetic and CHF patients as per practice guidelines 12. Monitor and maintains refrigerator temperature logs and biomed expirations for medical equipment 13. Participates in daily huddles with MDs and other clinical team members to review daily care management patient lists and discuss patient's treatment plan & management 14. Performs age-specific screenings for Activities of Daily Living, Falls, Alcohol, Tobacco, and/or Depression screenings annually for every patient 15. Enters data into the electronic medical record related to medication adherence, diabetes control, foot/eye exam compliance for provider review and reconciliation 16. Assists providers with distributing educational materials to patients 17. Assists Population Health Management team by verifying patient has completed their colorectal screening, breast screening, cervical cancer screening and other pertinent preventive screens. 18. Retrieves information regarding hospital admission and Emergency Room visits from outside facilities for evaluation by provider for transition of care coordination 19. Assists providers with follow-up related to patients on Coumadin therapy 20. Assists at the front desk as needed: answer telephones, register patients, discharge patients, schedule appointments, copy charts, schedule ancillary tests, and follow up on overdue diagnostic test results. 21. Follows Primary Care Medical Home and MACRA protocols for designated role 22. Performs all other job related functions as assigned. Position Overview The Registered Nurse (RN) Supervisor plays a key leadership role in managing nursing operations during assigned shifts at Constant Care Agency’s partner nursing facilities in the Bronx. The RN Supervisor ensures that residents receive safe, high-quality care consistent with the standards of practice, regulatory requirements, and the mission of Constant Care Agency. This position requires strong clinical judgment, supervisory ability, and compassion for the elderly and long-term care population. Primary Responsibilities Clinical Supervision Oversee all nursing and direct care staff (RNs, LPNs, CNAs) on assigned units or shifts. Ensure timely and accurate delivery of nursing care in accordance with physicians’ orders, facility policies, and resident care plans. Monitor residents’ conditions; respond promptly to changes in status and initiate appropriate interventions or physician notifications. Conduct walking rounds at the beginning and end of each shift; verify continuity of care and accurate reporting. Oversee medication administration and treatments to ensure safety and compliance. Staff Leadership & Management Assign, direct, and evaluate the performance of nursing staff; delegate tasks appropriately based on scope of practice. Provide mentorship, education, and on-the-job training for new or less experienced staff. Ensure adequate staffing levels; manage call-ins and replace staff as needed in collaboration with the staffing coordinator. Promote teamwork, professionalism, and accountability across the care team. Regulatory Compliance Ensure nursing practices comply with New York State DOH , CMS , and facility policies . Conduct chart audits to confirm accurate, complete documentation of resident care and progress notes. Participate in QA/QI programs, infection control audits, and safety committees. Prepare for and participate in DOH inspections and surveys. Maintain resident confidentiality and HIPAA compliance at all times. Emergency Response Serve as the on-site clinical authority during assigned shifts. Respond to medical emergencies, perform immediate assessments, and direct staff response. Communicate critical incidents to the DON or Administrator on call. Resident & Family Relations Foster compassionate, resident-centered care and ensure residents’ dignity and rights are upheld. Communicate effectively with residents and families regarding care issues or changes in condition. Participate in care plan meetings when required. Qualifications Licensure: Current, unencumbered Registered Nurse (RN) license in New York State . Experience: Minimum 2–3 years of RN experience , with at least 1 year in a long-term care or skilled nursing facility . Prior supervisory experience strongly preferred. Education: Graduate of an accredited nursing program (BSN preferred). Certifications: BLS required; ACLS preferred. Knowledge: Comprehensive understanding of NYS DOH and CMS nursing home regulations. Familiarity with MDS documentation, infection control, fall prevention, and pressure injury protocols. Skills: Strong clinical assessment and problem-solving ability. Excellent leadership, delegation, and interpersonal communication skills. Ability to handle emergencies calmly and professionally. Proficiency in electronic health records (e.g., PointClickCare, SigmaCare). Work Conditions Work performed in a skilled nursing facility environment. For Nurse-Family Partnership (NFP) only: Provides health education, social support and community linkages to meet the needs of pregnant and postpartum, first-time mothers and their infants. Also responsible for adhering to NFP national guidelines. • Assesses the physical, social and psychological needs and environmental conditions of the patient/client through professional knowledge, skills of observation, interviewing, and physical examination to develop a plan of care. • Understands, interprets, and implements reimbursement protocols; determines the billing source for patient/client care. Consults with management on complex/unique reimbursement cases. • Acts as the coordinator of care for the multidisciplinary team by providing and managing the care needs of patients/clients which includes making/confirming referrals to other disciplines/community agencies and ordering supplies/services and ensuring they are delivered as needed. • Documents content and process of all services provided in accordance with government regulations/requirements, and VNS Health policies, practices, and procedures, as well as NFP requirements as appropriate.
